摘　　要：目的找出胃肠术后血浆白蛋白水平与并发症的关系并为临床工作提供有用信息。方法前瞻性观察连续159例胃肠手术后患者,检测并统计患者的术前及术后0,1,2,3,5,7d白蛋白水平与住院期间并发症的关系。结果有并发症组与无并发症组患者的术后血浆白蛋白水平存在差异(P<0.05);logistic回归分析显示术后第3天的血浆白蛋白水平是住院期间并发症的独立预后因素,ROC曲线和配对卡方检验显示,综合敏感性与特异性考虑,术后第3天血浆白蛋白水平<30g/L是住院期间并发症的最佳预测指标。结论术后第3天血浆白蛋白水平是住院期间并发症的良好预测指标。术后第3天血浆白蛋白水平<30g/L可作为胃肠术后住院期间并发症的预测值。Objective Researches on association of postoperative serum albumin level and complications following gastrointestinal surgery are rare. The aim of this study is to find out this association and provide useful information for clinical work.Methods A prospective observational study of 159 consecutive patients who underwent gastrointestinal surgery for malignant disease was performed. Patients＇ pre and postoperative （days 0, 1,2,3,5 and 7） serum albumin levels were measured and statistically tested for the association of albumin level and in-hospital complications. Results Postoperative serum albumin levels were different between patients of complication group and complication-free group （P〈0.05）; logistic regression analysis shows serum albumin level on the third postoperative day was the independent predictive factor for in-hospital complications. ROC curve and paired chi-square test revealed Serum albumin level〈30 g/L on the third postoperative day was the best prognostic value for in-hospital complications, comprehensively considering the sensitivity and specificity. Conclusions Serum albumin level on the third postoperative day is a good predictor of surgical outcome. Serum albumin level〈30 g/L on the third postoperative day may be used as a prognostic value for in-hospital complications following gastrointestinal surgery.
