木瓜蛋白酶诱导骨关节炎模型中病理指标的动态变化

摘　　要：为探讨木瓜蛋白酶诱导骨关节炎（osteoarthritis，OA）模型中病理指标的动态变化，于第1、4、7d注射木瓜蛋白酶诱导OA模型，由第10d开始，分别于造模结束后第1、2、3周采用Elisa法测关节滑液中IL-1B含量，游标卡尺测肿胀程度，测痛仪测压痛阈值，观察脚掌印记测行走疼痛记分，大体观察软骨表面损伤并评分，甲苯胺蓝、沙黄0染色以观察软骨组织病理学变化并进行了Mankin’s评分．结果表明：模型组第1、2、3周的IL-1B含量升高，第2周达峰值；第1周肿大严重，2、3周回落；第1周痛阈降低，2、3周升高；各周疼痛记分逐渐升高；各周大体观察评分和Mankin’s评分逐渐升高．说明木瓜蛋白酶诱导OA模型初期（1—2周），出现伴有IL·1B增多、关节肿大、痛阈降低的急性炎症反应，第2周后炎症缓解，但软骨损伤仍不断加重，并反映为步态变化．To investigate the dynamic change of pathological indicators during the formation of papain-induced rat osteoarthritis （OA）, an OA model of rats was used to examine limb swelling by vernier caliper, pain threshold by algesimeter, pain scoring of walking by observing the photo of imprinted paw, the concentration of IL-113 in articular synovia by Elisa, damage of knee articular cartilage by general observation score, and general histopathology of knee articular cartilage by Mankin score through observing toluidine blue and safranin O staining. The results indicated that the swelling of the limbs injected with papain increased seriously at the end of the first week, then decreased at the end of the second and third week. The pain threshold decreased at the end of first week then increased at the end of second and third week. The pain score increased gradually at the end of first, second and third week. The concentration of IL-113 elevated at the end of first, second and third week, plateaued at the second week. The general observation score and Mankin score increased gradually at the end of first, second and third week. So the acute inflammatory response appeared on the initial stage of papain-indueed OA model （first and second week）, with IL-113 increasing, arthrosis swelling, pain threshold reducing. At the end of second week, inflammation was relieved, but the damage of articular cartilage got worse, as shown by the change of walking style.
