开窗减压引流术治疗下颌骨巨大囊性病变

摘　　要：目的评估开窗减压引流术在下颌骨巨大囊性病变治疗中的临床应用价值。方法回顾分析4例下颌骨巨大囊性病变(角化囊性瘤1例、多囊型成釉细胞瘤2例、单囊型成釉细胞瘤1例),均行开窗减压引流术,临床随访行全颌曲面断层片检查,进行颌面部影像对比分析。结果 4例下颌骨巨大囊性病变囊腔均缩小、新骨形成、颌骨形态恢复,避免了下颌骨切除,牙颌功能保留,无下唇麻木,其中2例已二期手术刮除残余囊壁,随诊1年无复发。结论开窗减压引流术,最大限度保留下颌骨形态和功能,是治疗下颌骨大型囊性病变的微创方法。Objective To evaluate the clinical value of decompression by open drainage in treating large mandible cystic lesions. Methods Four cases of large mandible cystic lesions were retrospectively analyzed, including one case of keratocystic tumor, two cases of multicystic ameloblastoma, one case of unicystic ameloblastoma, were treated by open drainage and decompression. Clinical follow- up panoramic radiographs was used to observe and comparatively analyze maxillofacial imaging. Results The giant cystic lesions shrinked, with new bone formation, jaw morphology recovery, avoiding mandible resection, dental function preservation, without lower lip numbness, in two cases, the second surgical curettage to residual wall, followed up for one year and no recurrence. Conclusion Open drainage, maximizes the retention of the morphology and function of the mandible is the minimally invasive approach for the treatment of large mandibular cystic lesions.
