杭州市社区老年人牙科焦虑得分及影响因素研究

摘　　要：目的了解杭州市社区老年人牙科焦虑得分情况并分析其影响因素。方法采用多阶段分层整群随机抽样法,确定600名研究对象,采用汉化Coarh′s牙科焦虑量表(Dental Anxiety Scale,DAS)对其牙科焦虑情况进行问卷调查,应用单因素方差分析、多元线性回归分析方法对研究对象的牙科焦虑得分情况及影响因素进行分析。结果收回问卷596份,应答率为99.3%。剔除无效问卷后,合格问卷573份,有效率为96.1%。杭州市社区老年人DAS得分为(7.51±3.18)分,高度牙科焦虑发生率为6.1%(35/573)(DAS得分≥13分的调查对象被认为患有高度牙科焦虑);单因素方差分析结果显示,牙科焦虑得分在不同性别、不同年龄组、不同月收入、不同疼痛耐受程度、自己的不同看牙经历、亲友的不同看牙经历以及不同口腔卫生状况的社区老年人之间差异有统计学意义(F值分别为9.45、18.24、33.34、38.66、12.62,P均<0.05);多元线性回归分析结果显示,对社区老年人DAS得分有显著影响的因素为疼痛耐受程度(P<0.01)、性别(P=0.002)、年龄(P=0.007)和月收入(P=0.017),女性、年龄偏轻、月收入低、疼痛耐受程度差的社区老人DAS得分较高。结论杭州市社区老年人高度牙科焦虑发生率较低,疼痛耐受程度、性别、年龄和月收入是杭州市社区老年人牙科焦虑得分的影响因素。ZHU Chen-xi;LIU Xin-gong;HE Fu-ming;LI Ying-zhou;WANG Xue-lin;The Gongshu Medical Association; Objective To understand the current status of dental anxiety（DA）and it＇s influencing factors among the elderly in communities of Hangzhou. Methods A total of 600seniors were randomly selected by multi-stage stratified cluster sampling method and surveyed using the Coarh＇s Dental Anxiety Scale（DAS）. One-way ANOVA and multiple linear regression analysis were used to analyze the data. Results 573out of 600questionnaires qualified for the study were retrieved.The average DAS score was 7.51±3.18.The prevalence of high DA was 6.1%.One-way ANOVA showed that the differences of DA scores were significant among seniors with gender,age,monthly income,pain tolerance,experience of dental care and oral hygiene（P〈0.05）.Meanwhile,multiple linear regression analysis revealed that pain tolerance,gender,age and monthly income were closely related to the DAS score.Usually,seniors of female with younger age,low income and poor pain tolerance were more likely to have higher scores of DA. Conclusions The prevalence of high DA is low among the elderly in communities of Hangzhou city.Pain tolerance,gender,age and monthly income are influencing factors for DA.
