广东省儿童青少年致死性伤害流行病学特征分析

摘　　要：目的了解广东省儿童青少年伤害死亡的流行病学特征，为制定相应干预措施提供参考依据。方法采取等比例分层整群随机抽样方法，在广东省抽取12个县（区）开展2004-2005年居民死因回顾性调查。结果广东省2004-2005年0～19岁儿童青少年伤害死亡率为19．9／10万，占该年龄组人群总死亡数的34．0％，是该年龄组的第1位死因；伤害死亡率农村高于城市，男性多于女性。其中0～4岁儿童伤害死亡率为22．6／10万，农村是城市的1．7倍，男、女差异无统计学意义；5～14岁儿童伤害死亡率为15．9／10万，农村是城市的1．5倍，男性是女性的1．6倍，溺水是0～14岁儿童的第1位伤害死因；15～19岁人群伤害死亡率为26．6／10万，农村是城市的1．4倍，男性是女性的2．5倍；交通伤害是15～19岁青少年的第1位伤害死因。广东省儿童青少年每年因伤害死亡导致的潜在寿命损失年（YPLL）为35．2万人年。结论广东省儿童青少年致死性伤害已成为严重的公共卫生问题，导致的疾病负担沉重。Objective To understand the epidemiological characteristics of fatal injury among children in Guangdong province, and to provide evidence for making related interventions. Methods Using proportionally stratified cluster sampling method, a representative sample of cases due to fatal injury during 2004 to 2005 was collected from 12 counties or districts in Guangdong Province. The indicators of data included mortality, standardized mortality and order of death causes. Results There were 993 deaths of fatal injury among children. The mortality of the injury was 19.9/100 000, accounting for 34.0% of the total mortality among children. Injury was the first leading cause of deaths in youth aged 0-19 years. Higher injury mortality was found in rural area and in girls. Injury deaths among children aged 0-4 years, 5-14 years and 15-19 years was 22.6/100 000, 15.9/100 000, 26.6/ 100 000, respectively. The leading causes of fatal injury were traffic accident among youth in 15-19 years old. Years of Potential Life Lost （YPLL） in injury were 352 000 person years among children in Guangdong province. Conclusion Fatal injury has been a serious public health problem among children in Guangdong province. It is important that effective interventions are developed and implemented to minimize the impact of fatal injury.
