贲门癌经腹切除术的疗效研究

摘　　要：目的探讨贲门癌经腹切口手术的治疗效果。方法选取我院2000年1月—2011年12月手术治疗胃底贲门癌患者79例,均行经腹切口手术。结果 79例患者术后均无死亡等严重并发症;所有患者的吻合器切除圈完整,食管切缘无癌细胞残留;平均住院时间为11d。41例患者出现程度不等的反流性食管炎。8例患者术后半年出现不同程度的吻合口狭窄,行吻合口气囊扩张术后康复。2例患者术后出现胸腔积液,予以胸腔穿刺抽液后康复。1例患者术后5d发生应激性溃疡,表现为消化道出血,经止血、抑酸等治疗后康复。1例患者在术后第7天出现吻合口瘘,采用禁食、持续胃肠减压后顺利康复。79例患者病理检查,各切缘均无癌细胞残留;淋巴结转移72例,其中贲门周围淋巴结转移68例,胃左动脉淋巴结转移30例。结论经腹手术比经胸手术优点多。经腹手术对贲门癌根治更彻底,并发症更好,适应证更广。Objective To investigate the therapeutic effect of abdominal incision operation of cardiac cancer. Meth- ods In our hospital from 2000 January to 2011 December operation treatment of cardia cancer in 79 cases, were treated with ab- dominal incision operation. Results All 79 patients without serious complications such as death ; All patients complete excision circle, the esophageal marginno residual cancer cells; The average hospitalization time was 11 d. 41 patients had different de- grees of reflux esophagitis. The first half of 8 cases of patients with different degrees of anastomotic stenosis, anastomotichealing for balloon dilatation after. 2 cases of patients with postoperative pleural effusion, pleural puncture afterrehabilitation to be. 1 patients after 5d occurred stress ulcer, manifested as gastrointestinal bleeding, bleeding, antiacid treatment after rehabilitation. 1 cases of anastomotic fistula occurred in seventh days after operation, with fasting, continuous gastrointestinal decompression after successful rehabilitation. 79 patients with pathological examination, the margins were no residual cancer; Lymph node me- tastasis in 72 cases, including cardiac peripherallymph node metastasis in 68 cases, left gastric arterylymph node metastasis in 30 cases. Conclusion Transabdominal operation than transthoracic operationadvantages. Radical transabdominal operation more thoroughly, complications of cardiac cancer better, indications and more.
