摘 要:目的探讨肺小腺癌中原位腺癌半定量量化分级和病灶中磨玻璃密度含量的相关性。方法收集超高分辨率CT(UHRCT)发现并具病理诊断腺癌的最大径≤2cm的肺部孤立性结节fGGO104例,用计算机软件测量肿瘤内GGO成分的百分比,并对腺癌中的AIS成分半定量测定分为4级:Ⅰ级为AIS含量≥90%病灶面积,Ⅱ级为AIS含量占约50oA~89%病灶面积,Ⅲ级为AIS含量占约10%~49%病灶面积,Ⅳ级为AIS含量〈10oA病灶面积。对两者相关性进行分析。结果AISI级病灶32例,GGO平均含量85.69%;II级病灶30例,GGO平均含量68.75%;Ⅲ级病灶32例,GGO平均含量51.57%;Ⅳ级病灶10例,GGO平均含量37.46%;除3组和4组间差异无统计学意义外(P=0.078),其他各组两两间GGO含量差异均有统计学意义(P〈O.05)。结论小腺癌中AIS含量与GGO含量正相关,但当AIs含量小于50%时,GG0含量与AIS含量没有明显相关性。Objective To investigate the correlation between ground glass opacity content and semi-quantitative classifi cation of adenocarcinoma in situ in small pulmonary adenocarcinoma. Methods There were 104 cases of solitary pulmona ry fGGO with pathological diagnosis of adenocarcinoma with the diameter ≤ 2 cm on the ultra high resolution CT (UHRCT) imagings, percentage of GGO component within the tumor was detected by a software, and the semi-quantita tive classification of AIS in small adenocareinoma is divided into 4 grades, AIS content greater than 90% focus area is grade I , AIS content accounted for about 50%~89% focus area is grade II, AIS content accounted for about 10%~ 49% focus area is gradeⅢ , AIS content 〈 10% focus area is grade IV. The correlation between the two groups is analysed. Results The average content of GGO among 32 grade I AIS lesions is 85. 69%, of which 30 grade II AIS lesions is 68.75%, of which 32 grade Ill AIS lesions is 51.57%, and of which 10 grade IV AIS lesions is 37.46% ; there were sta tistically significant differences between the content of GGO and semi-quantitative classification of bronchoalveolar carcino ma in small adenocarcinoma ( P 〈0.05) in all groups but not between the group 3, and group 4 ( P = 0. 078). Conclusion There was a positive correlation between the content of AIS and the content of GGO in the small adenocarcinoma, but, when the content of AIS is less than 50%, there was no obvious correlation.
