阴道三维能量多普勒超声成像技术在早期宫颈癌及癌前病变中的应用

摘　　要：目的评价经阴道三维能量多普勒超声成像技术在早期宫颈癌及宫颈上皮瘤样病变(CIN)血管检测中的应用价值。方法随机选取2011年5月~2013年1月我院诊治的病例共160例,分为三组:早期宫颈癌(Ia期~IIa期)43例(宫颈癌组);高度宫颈上皮内瘤样病变(CINII^CINIII累及腺体)57例(CIN组)及60例慢性宫颈炎(对照组)进行阴道三维能量多普勒超声检查,并测定肿块内或宫颈内的血管形成指数(VI)、血流指数(FI)、血管形成-血流指数(VFI)三项血管参数值,比较三组间血管参数值的差异。结果宫颈癌组VI、FI、VFI均明显高于CIN组及对照组,差异有统计学意义(P<0.05),CIN组VI、VFI高于对照组,差异有统计学意义(P<0.05),而血管参数FI测值两者比较差异无统计学意义(P>0.05)。结论阴道三维能量多普勒超声能客观地反映宫颈的血供状态,可为早期宫颈癌及CIN的超声诊断提供重要参考信息,具有一定的应用价值。Objective To evaluate intratumoral vascularization in early-stage cervical cancer by transvaginal three dimen sional （3D） power Doppler sonography. Methods We randomly selected a total of 160 cases in the diagnosis and treat ment of Maternity and child health care hospital of xuzhou city, from may 2011 to january 2013, and divided into three groups： forty three patients with early-stage cervical cancer （cervical cancer group）, fifty-seven patients with CIN II～CIN Ill involving gland （CIN group） and sixty chronic cervicitis （control group） were studied by transvagina] 3D power Doppler sonography. The vascularization index（Vl）, flow index（FI） and vascularization-flow index（VFI） within the tumor or cervix were recorded and used for analysis. Vascular blood flow indices of cervical cancer were compared with those of CIN and normal control. The relationships between parameters were analyzed. Results VI, FI and VFI in cervical cancer group were significantly higher than those in CIN group and control group （ P 〈0.05）. VI and VFI in CIN group were al so significantly higher than those in control group （ P 〈0.05）. FI between CIN group and control group has no significant difference （ P 〉0.05）. Conclusion Transvaginal 3D power Doppler sonography can objectively reflect the state of blood supply of the cervix, which provides important reference information for diagnosing early-stage cervical cancer and CIN and thus has great application value.
