中心静脉导管在恶性胸腔积液治疗中的应用

摘　　要：目的:探讨中心静脉导管留置胸膜腔在恶性胸腔积液治疗中的应用价值。方法:收集临床已明确诊断的中或大量恶性胸腔积液患者63例,进行回顾性分析。其中行中心静脉导管留置胸腔引流的40例为观察组,行常规分次胸腔穿刺抽液的23例为对照组。比较两组患者的近期疗效、不良反应、并发症、穿刺次数、积液吸收天数等。结果:观察组完全缓解率52.5%(21/40),部分缓解率40%(16/40),总有效率92.5%(37/40);对照组完全缓解率34.8%(8/23),部分缓解率39.1%(9/23),总有效率73.9%(17/23)。观察组积液吸收天数5-22天,对照组14-36天(不包括无效病例)。胸膜增厚发生率观察组2.5%(1/40),对照组17.4%(4/23)。胸膜疼痛发生率观察组2.5%(1/40),对照组13%(3/23)。两组治疗效果比较有显著差异。结论:中心静脉导管胸腔留置技术在恶性胸腔积液的治疗中具有操作简便、创伤小、安全可靠、并发症少,可长时间留置,便于注药治疗,且疗效明显优于常规穿刺方法。可作为恶性胸腔积液的常规治疗手段。Objective：To investigate and evaluate the effect and clinical significance of central vein catheter closed drainage in the treatment of malignancy pleural effusion. Methods： For 40 patients who suffered from moderate to massive pleural effusion,we applied central vein catheter to get close drainage for treatment and observe the recent treatment. Results： Complication, disappear time, absorption ratio, side reactions of the pleural effusion were contrasted with the control group ,which include 23 patients who suffered from moderate to massive pleural effusion with traditional method to drain pleural effusion. In observation group, complete response rate was 52.5% （21/40）, part response rate 40% （16/40） ,the total effective rate 92.5% （37/40）. In control group complete remission rate was 34.8% （8/ 23 ）, part response rate 39.1% （9/23）, the total effective rate 73.9% （17/23）. The incidence of pleural thickening and pleural pain were 2.5 % （1/40 ） vs 17.4% （4/23）and 2.5 % （1/40） vs 13 % （3/23）. The indexes between the two groups had significant differences. Conclusion：Applying central vein catheter to close drainage for treatment of moderate to massive malignancy pleural effusion is safe and effective, simple and convenient, has fewer side reactions.
