摘 要:目的探索硬膜外腔注射蛇床子素对髓核致坐骨神经痛大鼠DRG中CGRPR1表达的影响,以阐明其镇痛机制。方法♂SD大鼠54只,随机分为3组:Blank组(12只)、Sham组(12只)、手术模型组(NP,30只),检测大鼠术前1d,术后1、3、7、14、21 d的50%MWT和TWL。Blank组和Sham组于术后7 d及NP组于术后1、3、7、14、21 d取术侧L5DRG,用Western blot方法检测CGRPR1蛋白相对浓度。另♂SD大鼠72只,随机分为4组(每组18只):NP组,DMSO组,蛇床子素组,CGRP8-37组。手术前后所有大鼠均进行疼痛行为学检测,NP组、DMSO组、蛇床子素组于术后7 d取材,通过Western blot检测CGRPR1蛋白相对浓度。结果NP组在术后各时间点50%MWT和TWL比术前和Sham组明显降低(P<0.05),术侧L5DRG中CGRPR1蛋白表达水平比Sham组明显升高(P<0.05)。与NP组和DMSO组相比,蛇床子素组50%MWT和TWL明显升高(P<0.05),同时术侧L5DRG中CGRPR1蛋白表达水平降低(P<0.05)。结论 CGRPR1表达上调参与介导髓核源性坐骨神经痛,蛇床子素可通过抑制CGRPR1蛋白表达而缓解痛觉过敏。Aim To explore the effect of epidural ap- plication of osthole on the expression of CGRPR 1 in rats with nucleus pulposus-induced sciatica. Methods Fifty-four male SD rats were randomized into 3 groups (18 rats per group ): blank group, sham group, and surgical model group (NP). 50% MWT and TWL were measured 1,3, 7, 14, 21 d after oper- ation and 1 d before operation. On postoperative day 7 blank group and sham group rats and on postoperative day 1, day 3, day 7, day 14, day 21 NP group rats were killed and the I_5 DRGs were harvested to detect CGRPR1 protein relative concentration using Western blot technique. Another 72 mate SD rats were random- ized into 4 groups ( 18 rats per group) : NP group, DMSO group, Osthole group, and CGRPS-37 group. Pain behavioral indicators in all rats were measured be- fore and after surgery. The L5 DRGs of rats in NP group, DMSO group, Osthole group on postoperative day 7 were harvested to detect CGRPR1 protein relative concentration using Western blot technique. Results The pain behavioral indicators of the NP group dropped significantly compared with the preoperative value and those of sham group ( P 〈 0. 05 ), and the expression level of CGRPR1 in 1.5 DRGs of NP group increased significantly (P 〈 0. 05 ). Compared with NP group and DMSO group, osthole could prevent the drop of pain-related behavioral indicators and reduce CGRPR1 protein expression ( P 〈 0. 05 ). Conclusion The up- regulation of CGRPR1 facilitates sciatica induced by nucleus pulposus and osthole can alleviate sciatica by reducing CGRPR1 protein expression.
