经尿道2μm激光汽化切除术治疗高龄表浅性膀胱肿瘤疗效观察

摘　　要：目的评估2μm激光汽化切除术治疗高龄表浅性膀胱肿瘤的疗效和安全性。方法39例高龄浅表性膀胱肿瘤患者均行经尿道膀胱肿瘤2μm激光汽化切除术，术后规律膀胱灌注吡柔比星，定期复查膀胱镜。观察手术时间、成功率及不良反应。结果39倒手术均获成功，手术时间（62．05±29．08）min。术中均无闭孔反射及膀胱穿孔等并发症发生。留置导尿（6．79±2．53）d。术后随访3～18个月，1例出院1周后膀胱出血并血块形成，经膀胱镜下2μm激光止血后治愈。2例膀胱肿瘤复发，经再次2μm激光治疗治愈。余患者至今未见复发。结论经尿道2μm激光汽化切除术治疗表浅性膀胱肿瘤具有出血少、围手术期安全性高、并发疽少等优点，而且操作安全简便，易于掌握，尤其适用于高龄患者。Objective It is to evaluate the efficacy and safety of RevoLix 2 μm continuous wave laser vaporization in treat- ment of elderly patients with non-muscle-invasive bladder tumor （NMIBT）. Methods 39 cases of elderly patients with NM1BT were underwent transurethral resection of bladder tumor by 2μm laser vaporizations, then were implemented with pirarubicin for intravesical instillation afteroperation, and regular cystoscopy. The operation time, operation success rate and adverse reac- tion were observed. Results All 39 cases got through perioperative period. The operative time was （62.05 ± 29.08） min. And there was no occurrence of obturator nerve reflection and bladder perforation. The indwelling catheter time was （6.79 ± 2.53 ） days. All patients were received follow-up for 3 to 18 months. Hemorrhage of bladder and blood clot formation was occurred in 1 case after 1 week, and was cured by cystoscope laser hemostasis. Bladder tumor recurrences were happened in 2 cases, and were cured by 2 μm laser vaporizations. All the others had no recurrence. Conclusion 2 μm laser vaporization was a superior surgery method for elderly patients with NMIBT, which can yield little blood loss, less pain, and quick recovery, with safety, simplicity and less adverse effects.
