摘  要:目的探讨屈光参差性近视患者的角膜地形图特性及其对病因学研究价值。方法选择在广西视光中心就诊的屈光参差性近视患者45例。使用OrbscanⅡz角膜地形图测量仪检测患者双眼角膜的形态学数据,包括角膜前、后表面的Diff值、Sim K值、角膜Jackson交叉柱镜负柱镜轴向(Jackson cross cylinder,JCC)、中央角膜厚度(central corneal thickness,CCT)、Kappa角等指标,按所对应眼屈光度数高低将每例患者的检测数据分别归入高度数组和低度数组。高度数组和低度数组检测指标的差异采用配对t检验比较,角膜地形图参数值在患者双眼间的关系,以及患者等效球镜值与角膜地形图参数值的关系采用直线相关分析。结果高度数组的角膜前表面Diff值为(0.006±0.006)mm,后表面Diff值为(0.025±0.006)mm,Sim K值为(0.96±0.61)D,JCC为95°±79°,J0为(-0.09±0.42)D,J45为(-0.04±0.38)D,CCT为(553±36)μm,Kappa角为4.2°±1.2°;而低度数组的角膜前表面Diff值为(0.007±0.005)mm,后表面Diff值为(0.026±0.007)mm,Sim K值为(1.03±0.64)D,JCC轴向为90°±77°,J0为(-0.03±0.50)D,J45为(-0.01±0.35)D,CCT为(555±32)μm,Kappa角为4.4°±1.4°。以上数据两组间比较,差异均无统计学意义(均为P>0.05)。角膜后表面Diff值、Sim K值、CCT、Kappa角等参数在两组间呈明显正相关(r=0.341、0.741、0.928、0.597,均为P<0.05)。而患者等效球镜值仅与Kappa角存在相关性(r=0.597,P<0.01)。结论本研究数据不支持角膜形态与屈光参差性近视相关联的猜测;尽管双眼屈光度存在着显著差异,但屈光参差性近视患者双眼在角膜地形图参数上存在很大的对称性和相关性。Objective To investigate the corneal topographic characteristics and the etiological role in anisomyopia. Methods Forty-five anisomyopic patients without amblyopia or strabismus were collected. A range of corneal topographic parameters were measured by Orbscan Ⅱ z in both eyes of each subject including Diff values of the anterior and posterior corneal surface, Sim K' s astigmatism, JCC, central corneal thick ness (CCT) ,and angle Kappa. Data were divided and allocated to more and less myopic eye groups according to the cycloplegic refraction of each subject. Interocular differ ences for the two groups were compared by paired t test. Relationships between both groups in corneal parameters, and the relationship of spherical equivalent with corneal parameters were also analyzed across all eyes. Results The Diff values of the anterior corneal surface were (0. 005 ± 0. 006 ) mm and ( 0. 007± 0. 005 ) nun, and those in the posterior corneal surface were (0.025 ± 0.006) mm and (0.025 ± 0.007) mm in the more and less myopic eye group, respectively. Additionally, Sire K' s astigmatism were (0.96 ±0.61 ) D and ( 1.03 ± 0.64 ) D, JCC were (95 ± 79) ~ and (90± 77 ) °, J0 values were ( - 0.09 ±0.42) D and ( - 0.03 ± 0.50 ) D, J45 values were ( - 0.04 ± 0.38 ) D and ( -0.01 ±0.35)D,CCT values were (553±36)um and (555 ±32) ixm,angle Kappa val ues were (4.2 ± 1.2) ° and (4.4± l. 4) o in the more and less myopic eye group, respec tively. No significant differences were observed in above corneal parameters between the two groups ( all P 〉 0.05 ). And significant positive correlation coefficients were not ed between the two groups in corneal posterior Diff value, Sim K' s astigmatism, CCT, and angle Kappa (r =0. 341,0.741,0. 928,0. 597 ;all P 〈0.05). Besides,the relationship between spherical equivalent and corneal morphological parameters was found only in angle Kappa(r = 0.597 ,P 〈 0.01 ). Conclusion The hypothesis of c
