慢性炎症性脱髓鞘性多发性神经根神经病自主神经损害对交感神经皮肤反应和自主神经症状量表评分的影响研究

摘　　要：目的探讨慢性炎症性脱髓鞘性多发性神经根神经病(CIDP)自主神经损害对交感神经皮肤反应(SSR)和自主神经症状量表(ASP)评分的影响。方法对2011年1月—2012年7月在浙江大学医学院附属第二医院(包括宁波市第二医院转诊病例)就诊的29例CIDP患者(病例组)和30例同期健康体检者(对照组)进行SSR检测及ASP评分,并将结果进行统计比较。结果 (1)SSR结果分析:病例组SSR异常率高于对照组(95.9%比0;χ2=36.291,P=0.000),SSR波幅低于对照组,差异均有统计学意义(P<0.05)。(2)ASP结果分析:病例组ASP的评分及评分异常率均高于对照组,且差异有统计学意义(P<0.05)。(3)CIDP患者的检查结果:CIDP患者表现有自主神经症状的异常率较SSR异常率及ASP评分异常率均低,且差异有统计学意义(P<0.05)。结论 CIDP自主神经损害时SSR和ASP评分均会发生明显改变,值得临床借鉴参考。Objective To investigate the impact of autonomic nervous system damage of chronic inflammatory demye-linating polyradiculoneuropathy( CIDP) on sympathetic skin response( SSR) and autonomic symptom profile( ASP) score.Methods Totally 29 CIDP patients treated in the Second Affiliated Hospital of Medicine School,Zhejiang University( including the referral cases from Ningbo NO.2 Hospital) between January 2011 and July 2012( CIDP group) and 30 healthy people receiv-ing regular physical examination in the same hospital( control group) were included in this study.SSR test and ASP were per-formed in all participants.Results The abnormal rate of SSR in CIDP group was higher than that of the control group( χ2= 36.291,P = 0.000).The wave amplitudes of SSR in CIDP group were lower than those of the control group( P < 0.05).The total score of ASP and its abnormal rate in CIDP group were significantly higher than those of the control group( P < 0.05).In CIDP group,the rate of CIDP patients with abnormal autonomic symptoms was significantly lower than the abnormal rates of SSR and ASP score( P < 0.05).Conclusion Both SSR and ASP score will be apparently changed in the CIDP patients with auto-nomic nerve dysfunction,therefore it may provide valuable reference for its diagnosis and treatment.
