摘 要:目的探索环磷酰胺与顺铂联合化疗时间节律性特征,从而确定最佳给药模式。方法对42例卵巢癌和恶性淋巴瘤患者随机分为6组,分别按昼夜6个节律点静脉给药,比较不同时间外周血象、肾功能、肝功能的变化,评价药物时间化疗的不良反应。结果上午9:00化疗组白细胞抑制率和淋巴细胞损伤最大,血肌酐、血尿素氮、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)显著下降;而17:00化疗组血象在正常范围,肾功能、肝功能损伤发生频率低。结论环磷酰胺与顺铂联合化疗存在时间节律性,17:00化疗为最佳给药模式。Objective To determine the optimal model of drug administration based on the characteristics of temporal rhythm of chemotherapy with cyclophosphamide and cisplatin. Methods Forty-two patients with ovarian cancer and malignant lymphoma were randomly assigned to 6 groups, based on 6 time points, for administration of medications intravenously. This entailed monitoring of the changes in peripheral hemogram, renal and liver function at different time points for appraisal of adverse reactions. Results Chemotherapy administered at 9:00 a.m. yielded the maximal leukocyte inhibitory rate, markedly impaired lymphocyte function and pronouncedly reduced levels of serum creatinine, blood urea nitrogen, alanine aminotransferase (AST) and aspartate aminotransferase (AST). However, chemotherapy given at 17:00 was associated with normal hemogram and minor incidence of impaired renal and liver function. Conclusion There is a temporal rhythm of chemotherapy with cyclophosphamide and cisplatin, with the optimal model of an administration at 17:00.
