摘　　要：本文对国内外社区老年慢性病连续护理的研究现状进行综述。目前社区老年慢性病连续护理的研究病种主要为五大慢性病,研究侧重于对社区老年慢性病连续护理的认知和需求的调查、模式和护理内容的探讨。我国社区连续护理存在体制不完善、医院与社区缺乏有效衔接等问题。This paper gave a review on the research status of the continuous nursing of community elderly patients with chronic diseases and pointed out the main research objects. The research focused on cognition and demand investigation, the mode and nursing content discussion. There are some problems such as incomplete system, lack of effective connection between hospital and community, in order to provide the reference for further research.
