低位腹主动脉球囊阻断在控制骶骨巨大肿瘤术中出血的应用

摘　　要：目的探讨骶骨巨大肿瘤切除术中腹主动脉球囊阻断对出血控制的效果。方法在骶骨肿瘤患者切除中采用腹主动脉球囊阻断术10例,回顾骶骨肿瘤11例应用传统的术前血管栓塞技术的临床资料,并进行比较。统计学处理采用方差分析t检验和χ2检验。结果腹主动脉球囊阻断组手术时间为(326±93.8)min,术中出血量为(3750.7±644.1)mL,有1例骶神经损伤。对照组手术时间为(370±69.3)min,术中出血量为(7388±706.99)m l,局部皮肤缺血性疼痛3例,骶神经损伤有1例,出现性功能障碍1例。两组比较手术时间t=1.2127,P>0.05,无显著性差异;术中出血量t=4.9446,P<0.001;并发症也有显著性差异。结论在骶骨肿瘤手术时,采用腹主动脉球囊阻断可以减少术中出血量及并发症。可有效控制骶骨巨大肿瘤切除术中的出血。Objective To explore the abdominal aorta balloon block on the effect of bleeding control in sacral tu- mor resection. Methods 10 cases of sacral tumor resection in patients with abdominal aorta balloon block, review the sa- crum tumors in 11 cases clinical data of traditional preoperative vascular embolization techniques applied, and the compari- son. Statistical processing using t test and x2 test of variance analysis. Results Abdominal aorta balloon block group op- eration time （326 ± 93.8） min, intraoperative blood loss （3750.7-644.1） for ml, 1 cases of sacral nerve damage. Control op- eration time （370 ±69.3） min, the bleeding of （7388± 706.99） ml, sacral nerve injury in 1 case, local skin ischemic pain （3 cases）, 1 case of sexual dysfunction. Comparing the two groups operating time t = 1.2127, P 〉 0.05, no significant differ- ence; lntraoperative blood loss, t = 4.9446, P 〈 0.001. Complications have significant difference. Conclusion In the sacral tumor surgeo,, USES the abdominal aorta balloon block can reduce the intraoperative blood loss and complication. Can effectively control hemorrhage of sacral tumor resection.
