摘  要:目的 探讨脑血管痉挛与血管壁细胞间粘附分子 1(ICAM 1)表达之间的关系。方法 对既往Willis环处二次注血方法加以改进 ,在每次注血后行脑膜和颅骨修补 ,通过脑血管造影测定基底动脉的直径来评价脑血管痉挛程度 ,用免疫组化的方法测定基底动脉壁ICAM 1的表达。结果 此模型产生了明显的脑血管痉挛效应 ,以蛛网膜下腔出血后 7天时最明显 ,11例有脑血管痉挛者血管壁全部有ICAM 1的表达 ,且随脑血管痉挛程度的加重血管壁ICAM 1的表达也随之增强 ,而 4例无脑血管痉挛者仅 1例有轻度表达。结论 蛛网膜下腔出血后脑血管痉挛的程度与血管壁ICAM 1表达程度之间存在着明显的相关性 。Objective To summarize the correlation between cerebral vasospasm(CVS) and expression of intercellular adhesion molecule 1(ICAM 1) in the artery wall.Methods The model of SAH was improved by mending dural and skull deficit after each injection of blood.The diameter of basal artery was detected by DSA.The expression of ICAM 1 was measured by immunohistochemical technique.Results The obvious CVS was induced in the model.The degree of CVS reached the top on 7th day following SAH.All 11 cases of CVS had the expression of ICAM 1 in the artery wall.With the severity of CVS,the expression of ICAM 1 in the artery wall increased. Only one the other 4 cases without CVS had light expression of ICAM1 .Conclusion The degree of CVS following SAH is related to the expression of ICAM1 in artery wall. It suggests that immunoinflammatory reaction may play an important part in the mechanism of CVS after SAH.
