产前超声对胎儿动脉导管瘤的诊断和鉴别诊断价值

摘　　要：目的探讨胎儿动脉导管瘤的产前超声诊断价值。方法选择2010年1月至2012年12月间妊娠晚期前来我院进行产前超声检查的孕妇,根据国际妇产科超声协会(ISUOG)胎儿心脏超声指南进行四切面再加上主动脉弓和动脉导管弓切面筛查,着重观察三血管气管切面和动脉导管弓切面上动脉导管的内径及走行。结果本组共检出106例胎儿动脉导管瘤,其超声心动图表现为:四腔心切面上心脏大小正常,右心室内径大于左心室内径;三血管气管切面和动脉导管弓切面上动脉导管走行迂曲,甚至迂曲呈蚯蚓状,动脉导管内径明显扩张,内径>8 mm,最宽达12 mm。结论产前超声能有效地检出胎儿动脉导管瘤,它对晚孕期鉴别诊断胎儿右心扩大的病因有着重要的价值。Objective To explore the role of fetal echocardiography in prenatal diagnosis and differential diagno- sis of fetal ductus arteriosus aneurysm. Methods The pregnant women had the routine ultrasound scan in the third trimester. Four cardiac views were routinely visualized, including the four-chamber, left and right ventricular outflow tract and three vessel and trachea （3VT） view, together with the longitudinal view of the ductal arch and aortic arch. The focus was put on observing the diameter and curvature of the ductus. Results One hundred and six cases of fetal ductus arteriosus aneurysm were detected in our study. The characteristics of fetal ductus arteriosus aneurysm were summarized. The size of the fetal heart is normal and right ventricle is larger than left ventricle on the foor-chamber view. The ductus arteriosus is obviously dilated （〉8 mm） and curved on the ductal arch and 3VT view. Conclusions Fetal echoeardiography is a valid tool to detect the fetal ductus arteriosus aneurysm, which plays an important role in differential diagnosis of fetal right ventricle dilatation anomalies.
