摘 要:[目的]为下伸肌支持带重建下胫腓前韧带提供解剖学依据。[方法]8侧成人下肢标本,对下伸肌支持带上下内侧束及下胫腓前韧带的形态及解剖学数据进行观测,2侧新鲜标本进行模拟手术试验。[结果]下伸肌支持带跟骨附着点宽度(2.81±0.73)cm,自跟骨附着点至胫骨附着点的长度(8.53±0.42)cm,自趾长伸肌纤维管外侧壁至胫骨附着点的长度(5.87±0.62)cm;下胫腓前韧带的厚度(0.46±0.09)cm,在外踝附着点宽度(2.03±0.46)cm,在胫骨附着点宽度(1.35±0.27)cm,下伸肌支持带上内侧束胫骨附着点至下胫腓前韧带腓骨附着点的最短长度(5.45±1.06)cm。[结论]下伸肌支持带上内侧束潜行转位重建下胫腓前韧带具有可行性,自跟骨附着点切取下伸肌支持带全长可反折加强重建下胫腓前韧带。[ Objective ] To provide anatomic basis of inferior extensor retinaculum used for reconstruction of anterior inferior tibiofibular ligament. [ Method ] Morphology and anatomy data of superior and inferior medial cord of inferior extensor retinacu-lum, and anterior inferior tibiofibular ligament were measured in 8 lower limbs of adult cadavers. Additionally, mimic surgery of reconstruction of anterior inferior tibiofibular ligament with transfer of inferior extensor retinaculum were conducted on other 2 fresh specimens of cadavers. [ Result] The width of attachment on calcaneus of inferior extensor retinaculum was (2.81 ± 0. 73 ) cm, and the length from the calcaneal insertion to the tibial attachment was ( 8.53 ± 0.42) cm. The length of the retinaculum from the fibous tube wall of the extensor digitorum longus to the tibial attachment was(5.87 ± 0.62)cm. Futhermore,the thick-ness of anterior inferior tibiofibular ligament was (0.46 ± 0.09 ) cm, and the width of the ligamentous insertion on the lateral mal-leolus and the tibia were (2.03 ± 0. 46) cm, and ( 1.35 ± 0.27) cm respectively. The shortest length from tibial attachment of me-dial bundle of the inferior extensor retinaculum to the anterior inferior tibiofibular ligament of fibula attachment point was (5. 45cm ± 1.06) cm. [ Conclusion] It is possible to reconstruct the anterior inferior tibiofibular ligament by transfer of medial bun-die of the inferior extensor retinaculum. Moreover, strengthened reconstruction of the anterior inferior tibiofibular ligament could be performed by calcaneal deattachment and transfer of the inferior extensor retinaculum.
