后腹腔镜肾癌根治术在早期肾癌中的应用价值

摘　　要：目的探讨后腹腔镜肾癌根治术在早期肾癌中的应用价值。方法回顾性分析本院收治的68例早期肾癌患者的临床资料,根据手术方法的不同分为后腹腔镜组和开放手术组,每组各34例,其中后腹腔镜组患者采用后腹腔镜肾癌根治术进行治疗,开放手术组患者采用开放性肾癌根治性切除术治疗,比较两组患者的手术情况和并发症发生情况。结果后腹腔镜组患者的手术时间和手术治疗费用显著高于开放手术组患者(P<0.05);后腹腔镜组患者的术中出血量、术后排气时间、下床活动时间、住院时间显著短于开放手术组患者(P<0.05);后腹腔镜组的总并发症发生率为8.82%,其中2例腹膜破裂、1例中转开腹,开放手术组总并发症发生率为5.88%,2例术后出血,两组并发症发生率差异无统计学意义(P>0.05)。结论后腹腔镜肾癌根治术治疗早期肾癌,术中出血量少,术后恢复快,并发症发生率低,但临床费用相对较高。Objective To investigate the application value for latter laparoscopic radical nephrectomy in early kidney cancer. Methods The clinical data of 68 cases with early kidney cancer was taken for retrospective analyzed.They were divided into the latter laparoscopic surgery group and the open surgery group according to different methods,and each group of 34 cases.The latter laparoscopic surgery group was taken with laparoscopic retroperitoneal laparoscopic radical nephrectomy surgical treatment,and the open surgery open group was taken with open radical nephreetomy for treat- ment.The surgery and complications for the two groups was compared respectively. Results The surgery time and surgi- cal treatment costs in the latter laparoscopic surgery group were significantly higher than that of the open surgery group （P〈0.05）.The blood loss,postoperative flatus,ambulation,hospitalization time in the latter laparoscopic group were signifi- cantly shorter than that of the the open surgery group （P〈0.05）.The total complication rate in the latter laparoscopic group was 8.82%,of which 2 cases were of peritoneal rupture,one case was of laparotomy.The total complication rate in the open surgery group was 5.88%,two cases were of postoperative bleeding,and there were no significant differences （P〉0.05）. Conclusion The latter laparoscopic radical nephrectomy in the treatment of early kidney cancer has the ad- vantages of less blood loss,rapid postoperative recovery,low complication rate,but the clinical costs are relatively high.
