CT和血清CA19-9联合检测在胰腺癌诊断中的临床意义研究

摘　　要：目的探讨CT联合血清CA19-9检测诊断胰腺癌的临床价值。方法回顾性分析119例胰腺占位性病变患者病历资料。所有患者均于术前行CT扫描及CA19-9检测。统计分析119例患者CT诊断及血清学诊断结果,并对单种检测结果及联合检测结果与病理诊断结果进行对比。结果 119例患者中经病理确诊为胰腺癌共80例。CT诊断为阳性病例共87例,其中70例确诊为胰腺癌。CA19-9诊断为阳性病例共76例,其中61例确诊为胰腺癌。联合检测为阳性结果共59例,其中55例确诊为胰腺癌。CT诊断灵敏度,特异度,符合率分别为87.50%,56.41%,77.31%,CA19-9诊断灵敏度,特异度,符合率分别为76.25%,61.54%,71.43%。联合检测诊断灵敏度,特异度,符合率分别为68.75%,89.74%,75.63%。结论联合检测可提高对胰腺癌诊断特异度。对胰腺癌诊断具有较高临床价值。Objective To discuss the significance of CT and serum CA19-9 joint detection in the diagnosis of pancreatic cancer. Methods Retrospectively analysed 119 cases of pancreas space-occupying lesions patients＇s medical records. All the patients were examined with CT scan and CA19-9 testing before the operation. Statistically analysed 119 cases＇ results of CT and CA19-9 testing and compared the single testing result, combined detection results with the pathological diagnosis. Results 80 cases were diagnosed with pancreatic cancer in the 119 cases according pathological diagnosis. There were 87 cases whose CT diagnosis was positive and 70 cases were diagnosed pancreatic cancer among the positive cases. There were 76 cases whose CA19-9 diagnosis was positive and 61 cases were diagnosed pancreatic cancer among the positive cases. 59 cases were positive in the combined detection and 55 cases were diagnosed pancreatic cancer among the positive cases. CT diagnostic sensitivity, specific degrees, coincidence rate re- spectively were 87.50%, 56.41%, 77.31% when the results in the CA19-9 were 76.25%, 61.54%, 71.43%, and in the combined detection those data were 68.75%, 89.74%, 75.63%. Conclusion Combined detection is useful to improve the specific degrees in the diagnosis of pancreatic cancer.It has high clinical value in the diagnosis of pancreatic cancer.
