龙氏正骨手法联合针刺治疗颈源性枕大神经痛的研究

摘　　要：目的探讨龙氏正骨手法联合针刺治疗颈源性枕大神经痛的疗效。方法将105例符合条件的颈源性枕大神经痛患者按就诊顺序随机分为龙氏正骨手法联合针刺治疗组(A组35例)、龙氏正骨手法治疗组(B组35例)、针刺治疗组(C组35例)。3组患者治疗前后分别进行简化McGill疼痛问卷量表评定,并分析其远期的临床效果。结果总有效率A组为100%,B组为85%,C组为82%。A组总有效率明显高于B、C 2组(P均<0.05),而B、C组间相比无显著性差异。在疼痛分级指数(PRI)、视觉模拟量表(VAS)及疼痛强度(PPI)评分上,A组与B、C 2组相比亦有显著性差异(P均<0.05)。A组6个月内的复发率明显低于B、C 2组(P均<0.05),而B、C组间相比无显著性差异。结论龙氏正骨手法结合针刺治疗颈源性枕大神经痛疗效优于单纯龙氏正骨手法治疗和单纯针刺治疗,且远期疗效显著。Objective It is to observe the curative effect of Longshi manipulation plus acupuncture on cervical greater oc- cipital neuralgia. Methods 105 patients with cervical greater occipital neuralgia were randomly divided into three groups ac- cording to the visiting orders ：35 patients in group A treated with Longshi manipulation plus acupuncture, 35 patients in group B treated with Longshi manipulation, and 35 patients in group C with acupuncture. Before and after treatment, the curative effects were assessed in the three groups with the simplified MeGill pain questionnaire scale, and the long - term clinical effect was analyzed. Results The total effective rate in group A was 100% , 85% in group B, 82 % in group C. The total effective rate of group A was significantly higher than that of the other two groups （P 〈 0.05 ）. And the difference was not significant between group B and group C （ P 〉 0.05 ）. In addition, compared the score of pain classification index （PRI） visual analog scale （VAS） and pain intensity （PPI） of patients in group A to group B and C, the differences were statistically significant （P〈0.05）. 6 months recurrence rate of group A was significantly lower than that of group B and C （P〈0.05）, and the difference between group B and C was not statistically significant （P 〉 0.05 ）. Conclusion Longshi manipulation plus acu- puncture have definite curative effect on cervical greater occipital neuralgia, which is better than simple Longshi manipulation and simple acupuncture, and forward curative effect is distinct.
