超声内镜引导下无水酒精瘤内注射治疗胰岛细胞瘤的观察研究

摘　　要：目的探索超声内镜(EUS)引导下无水酒精瘤内注射治疗胰岛素瘤的安全性及有效性。方法 1例反复低血糖发作的48岁女性患者,经实验室检查、EUS、EUS-细针吸(FNA)确诊为胰岛素瘤。行超声内镜引导下无水酒精瘤内注射治疗,共向瘤内注射98%无水酒精0.5 mL。结果患者术后未再有低血糖发作,实验室检查证实获得完全缓解,术后第6天患者出院。结论对于不能行外科手术治疗的患者,微创的超声内镜引导下无水酒精瘤内注射可作为一种选择。Objective To explore the efficacy and safety of endoscopic ultrasound （EUS）-guided alcohol ablation of insulinoma. Method A 48-year-old woman was referred with typical symptoms of an insulinoma. Diagnosis was confirmed by laboratory findings, EUS, and EUS-guided FNA. EUS-guided alcohol injection was performed with a total of 0.5mL 98% ethanol injected into the tumor. Results No further hypoglycemic episodes was observed after the treatment, laboratory test indicated completed remission, and the patient was discharged 6 days after operation. Conclusions Minimally invasive EUS-guided alcohol ablation may be an alternative for patients with insulinomas in whom surgery is not feasible.
