管状吻合器在右颈胸腹三切口食管上段癌手术中的应用

摘　　要：目的总结右颈胸腹三切口食管胃颈部机械吻合治疗食管上段癌的临床经验。方法对我科2008~2012年66例胸上段食管癌,经右胸前外侧第4肋间切口、上腹、右颈三切口切除食管癌,用国产一次性使用管状吻合器行食管胃颈部机械吻合术。结果 66例手术均获成功,术后围手术期无死亡病例,无吻合口瘘病例,右侧喉返神经损伤3例（4.5%）。结论右颈胸腹三切口食管上段癌切除术,食管胃颈部管状吻合器吻合,操作简便,易于掌握及标准化,手术时间缩短,吻合口瘘发生率低。【Objectives】To summarize the experience of usage of circular stapler in 3 cuts on the right neck, chest and abdomen operation for upper esophageal carcinomas.【Methods】66 cases of upper esophageal carcinomas from 2008 to 2012 in our clinic were undergone the 3 cuts on the neck, chest and abdomen operation with domestic dis- posable circular stapler. 【Results】All the 66 operations were complicated successfully, without death and anasto- motic leakage. 3 cases（4.5%） of recurrent laryngeal nerve injury occurred. 【Conclusions】The 3 cuts on the right neck, chest and abdomen operation for upper esophageal carcinomas is uncomplicated to be carried out and stan- dardized, it can decrease the time of operation and the modality of anastomotic leakage.
