经尿道选择性绿激光前列腺汽化术与前列腺电切术联合全雄激素阻断治疗晚期前列腺癌的临床疗效比较

摘　　要：目的比较经尿道选择性绿激光前列腺汽化术与前列腺电切术治疗晚期前列腺癌的临床疗效。方法将214例前列腺癌患者随机分为经尿道选择性绿激光前列腺汽化术(PVP)组和前列腺电切术(TURP)组,各107例。分别行PVP术与TURP术。观察两组患者手术时间、住院时间、保留导尿管时间、术中术后出血、IPSS评分、最大尿流率及并发症。结果两组比较,手术时间无显著差异(P>0.05);保留导尿管时间有显著差异(P<0.05);两组术前术后QOL、Qmax均无显著差异(P>0.05)。术后尿道刺激症、继发性出血、电切综合征、尿失禁等并发症方面无显著差异(P>0.05)。结论 PVP术治疗晚期前列腺癌安全有效,术中出血少、恢复快、症状改善快、并发症少。【Objective】To compare the efficacy of green-light photo-selective vaporization of prostate（PVP） and transuretharal resection of prostate（TURP）.【Methods】A total of 214 cases of advanced prostate cancer were divid- ed into 2 groups; of them 107 cases underwent PVP and 107, TURP. The operative time, catheterization time, blood loss, IPSS, Qmax and complications were observed. 【Results】Comparison between PVP group and TURP group in- cluded the following： Two groups were similar in the operating time（P 0.05）. Remarkable differences were found in length of hospital stay after operation, catheterization time. No significant differences were found in QOL and Qmax（P 0.05）.【Conclusions】The PVP is safe and efficacious for older patients with advanced prostate cancer, with ad- vantages of less blood loss, better tolerance and few complications.
