B超及彩色多谱勒超声成像在眼后段外伤中的诊断应用

摘　　要：目的　评价B超和彩色多谱勒超声成像 (CDI)在眼后段外伤中的诊断价值。方法　用B超和CDI对连续 39例 45眼进行检查 ,B超对眼内异物的检出率与CT和X线检查结果进行比较 ,进行 2×k检验 ;B超检查描述玻璃体和球壁的详细情况 ,CDI检测眼动脉 (OA)、睫状后短动脉 (PCAs)、视网膜中央动脉 (CRA)、收缩期峰值速度 (PSV)、时间平均最大血流速度(TAMX )及其阻力指数 (RI) ,与健康眼进行比较 ,记录各参数的变化幅度和下降率 ,进行组间t检验。结果　B超对眼内异物的检出率为 80 % ,与CT结果一致 ,二者无显著差异 (P =0 .0 0 0 ) ,B超结合A超更有效区分眼内异物 ,了解玻璃体状况 ,PSV、TAMX在各组均有不同程度下降 ,在合并视网膜脱离或脉络膜脱离的病例中下降显著 ,有统计学显著差异 (P <0 0 5 )。结论　B超结合CDI检查对眼后段外伤的诊断有积极的作用 。To evaluate the role of B scan ultrasonography and colour dopplor imaging (CDI) in the detection of complicated posterior segment eye trauma with media opacities. Methods Contineous 39 cases of 45 eyes, with serious ocular trauma and not detected derectly through the opacitic media, were examed by B scan ultrasonography and CDI measurements, which results were compared with that of CT and X ray radiography, the peak systolic velocity(PSV),time averaged mapeak velocity(TAMX) and resisitance index(RI) of the posterior ciliary artiery(PCAs) and central retinal artiery (CRA) velocities were recorded and compared with that of healthy eyes. 2× k and t text were used to statistically analysis the results.Results The detection rate of B scan ultrasonography for the diagnosis of foreign body in the eye were consistent with that of CT eximanation, B scan recorded the details of the opacitic vitreous, CDI revealed that the PSV and TAMX the CRA and PCAs of the traumatic eyes with retinal detachment and choroidal detachment were obvious decendant than the controls(both P <0.05),especially in early bulbi atrophy. Conclusions B scan and CDI measurements were available in the detection and treatment induction of complicated eye trauma.
