摘 要:目的通过对黑龙江省哈尔滨市各类型初诊白血病患者的年龄和性别进行统计分析,探讨该地区白血病的流行分布特点。方法选取2010年1月至2011年12月间哈尔滨血液病肿瘤研究所和哈尔滨医科大学附属各医院收治的1427例各类型白血病初诊患者的骨髓穿刺结果,结合流式细胞术、骨髓活检、分子生物学检测和染色体分析对患者进行白血病确诊并分型。结果急性白血病患者1059例,占74.2%,平均发病年龄(33±19.8)岁。其中髓系白血病(AML)患者742例,占70.1%,平均发病年龄(38±18.0)岁;急性淋巴细胞白血病(ALL)患者317例,占29.9%,平均发病年龄(21±18.7)岁。慢性白血病患者368例,占25.8%,平均发病年龄(47±16.8)岁。在慢性粒-单核细胞白血病和21~40岁年龄组中,急性白血病女性患者居多,而其他各组男性白血病患者居多。所有白血病患者中,41~60岁年龄组分布最高。急性白血病患者主要分布于0~20岁年龄组,且随着年龄增长,分布比例逐渐减低。而慢性白血病主要分布于41~60岁年龄组,且在0~60岁范围内,慢性白血病的分布比例随着年龄的增长逐渐增加。结论黑龙江省哈尔滨市地区白血病患者以中老年(41—60岁)男性居多,急性白血病患者比例明显高于慢性白血病且多分布于青少年,白血病各亚型的分布与年龄有一定的相关性。Objective To analyze the relationship of age and sex of leukemia cases and the distrib- utive characteristics in Harbin city. Methods We retrospectively summarized all the cases collected from Institute of Harbin Hematology &Oncology and all the Affiliated Hospital of Harbin Medical University between January 2010 and December 2011 to classify the leukemia type by bone marrow aspiration, flow cytometry, bone marrow biopsy, molecular biology and chromosome analysis. Results A total of 1427 leukemia cases were recorded during the two years, the number of acute leukemia (n = 1059) patients accounted for 74. 2% and the mean age was (33 ± 19.8) years. In which, the number of acute myeloid leukemia (AML) patients accounted for 70. 1% and the mean age was (38 ± 18. 0) years. The number of acute lymphatic leukemia (ALL) patients accounted for 29. 9% and the mean age was (21 ± 18. 7) years. The number of chronic leukemia (n =368) patients accounted for 25.8%, the mean age was (47 ± 16. 8) years. There was a male predominance in these cases except for those chronic myelomonocytic leukemia (CMML) cases and acute leukemia cases in range of 21-40 years group. Of all the cases, the patients ranged 41-60 years accounted for 31.7%. Acute leukemia was more common in the range of 0-20 years and the rate was decreased with the growth of age. While chronic leukemia was dominated in the range of 41-60 years, and the rate was increased with the growth of age between 0-60 years. The mean age of acute leukemia patients were significantly younger than those with chronic leukemia. Conclusions The leukemia patients in Harbin city are dominated in middle-aged and male group. Acute leukemia has a higher rate than chronic leukemia and is more common in adolescent. The association is observed between the distribution of leukemia subtypes and the age.
