优质护理服务模式对肝硬化并发肝性脑病患者预后的影响

摘　　要：目的:探讨实施优质护理服务模式对肝硬化并发肝性脑病(hepatic encephalopathy,HE)患者预后的影响。方法:将以HE为主要临床症状的57例患者随机分为2组,对照组31例给予HE常规护理,干预组26例在对照组基础上实施优质护理服务模式,即实施提前落实告知义务、心理疏导及安全防范教育、预见性护理评估及预防措施、健康教育、出院指导、健康随访等护理干预措施。比较2组护理满意度及预后(治疗好转率和恶化病死率)情况。结果:干预组治疗好转21例,恶化病死5例;对照组治疗好转17例,恶化病死14例;干预组治疗好转率高于对照组(P<0.05)。结论:实施优质护理服务模式对融洽护患关系、增加患者满意度、改善HE患者的预后、提高其生存质量起到非常关键的作用。Objective：To explore the influence of high quality nursing service mode on the prognosis of patients with liver cirrhosis combined with hepatic encephalopathy（HE）. Methods：Fifty-seven patients with the clinical symptoms of HE were randomly divided into 2 groups. Thirty-one cases in the control group received the routine care and 26 cases in the treatment group were given high quality nursing service in addition to the routine care of HE, which is the implemenation of nursing eare interventions including the oluty to inform ahead of time, psychological counseling, security education preolictive care assessment and prevention, dischange quidance, and a number of post-discharge services such as health education and follow-up. The satisfaction degree and prognosis including the rate of improvement and fatality were compared between the two groups. Results： Twenty-one cases improved and 5 died in the treatment group ,while 17 cases improved and 14 died in the control group. The improvement rate of the treatment group was superior to that of the control group（ P 〈 0.05 ）. Conclusions： High quality nursing service mode plays an important role in harmonizing the nurse-patient relationship, enhancing the patients＇ satisfaction degree, and improving the prognosis and life quality of the HE patients.
