摘 要:目的分析不同浸润类型的胃癌患者幽门螺旋杆菌(Helicobacter pylori,HP)感染情况、病灶组织中环氧酶-2(cyclo-oxygen-ase-2,COX-2)和T淋巴瘤侵袭转移诱导因子(T-lymphomin vasion and metastasis 1,Tiam 1)表达情况以及分析HP感染情况与病灶组织COX-2和Tiam 1表达的相关性。方法 94例胃癌患者纳入本研究,且以正常患者24例为对照组。苏木精-伊红染色法(hematoxylin-eosin staining,HE)和血清HP抗体检测来评价HP的感染情况,用荧光定量聚合酶链式反应、蛋白质印迹法和免疫组化来分析病灶组织,两个结果最终进行回归分析,得出COX-2和Tiam 1与HP感染的相关性。结果胃癌T1患者平均HP抗体浓度为(10.21±2.11)μg/ml,胃癌T2患者平均HP抗体浓度为(19.09±3.61)μg/ml,胃癌T3患者平均HP抗体浓度为(26.82±2.04)μg/ml,胃癌T4患者平均HP抗体浓度为(34.51±4.21)μg/ml。随着血清HP抗体浓度的增加,患者病灶组织的Tiam 1 mRNA、蛋白表达量和IOD值逐渐上升。COX-2和Tiam 1蛋白表达量与血清HP抗体浓度呈正相关性(酌2=0.7485和酌2=0.8084)。结论 HP抗体浓度与COX-2或Tiam1的蛋白表达量具有正相关性,HP抗体浓度检测、COX-2和Tiam 1表达检测具有潜在的评价胃癌浸润程度的临床价值。Objective To analyze the Helicobacter pylori infection situation of different infiltration types of gastric carcinoma and the expression state of COX-2 and Tiara 1 in the tissue of the carcinoma, then analyze the correlation between infection situation of Helicobacter pylori and the expression state of COX-2 and Tiara 1. Methods 94 patients were included in this study, with 24 health volunteers as a control group. Hematoxylin-eosin staining (hematoxylin-eosin staining, HE) and the serum HP antibody test were used to evaluate HP in- fection situation, and fluorescence quantitative PCR, Western blot and immunohistochemistry were used to analyze the COX-2 and Tiam 1 mRNA and protein expression in lesion tissues. And regression analysis was used to analyze the correlation between COX-2 and Tiam 1 with HP infections. Results The average HP antibody concentration of T1 gastric cancer patients was ( 10.21 ± 2.11 ) μg/ml, T2 was ( 19.09 ± 3.61 ) μg/ml, T3 was (26.82 ± 2.04) μ g/ml, and T4 was (34.51 ± 4.21 ) μg/ml. With the increase of HP antibody concentration in serum of the patients, the expression amount of Tiara 1 mRNA, protein and the value of IOD also increased gradually. The expression amount of COX-2 and Tiara 1 were positively correlated with the HP antibody concentration in serum (R2=0.7485 and R2=0.8084). Conclusions The HP antibody concentration was positively correlated with the expression amount of COX-2 and Tiara 1. The detection of HP antibody concentration, COX-2 and Tiara 1 expression have potential clinieal value on evaluating the degrees of gastric cancer infiltration.
