摘 要:目的:探讨bcl-2与nm23-h1基因在乳腺癌中的表达情况及其相关性,并探讨其潜在临床意义。方法:采用免疫组化SP法观察85例乳腺癌手术切除标本及正常乳腺组织石蜡切片中bcl-2和nm23-h1的表达,并进行分析。结果:63.53%(54/85)肿瘤呈bcl-2阳性表达,bcl-2表达与肿瘤淋巴结转移及TNM分期有关(P值均<0.05)。40.00%(34/85)肿瘤呈nm23-h1阴性表达,nm23-h1阴性表达与肿瘤大小、淋巴结转移、TNM分期有关(P值分别为0.032、0.001和0.001)。单因素分析显示:bcl-2与nm23-h1表达为影响癌细胞淋巴转移的主要因素(OR值分别为6.985、0.014,P<0.001)。结论:bcl-2基因高表达与nm23-h1基因的低表达可能与乳腺癌的发生、转移密切相关,其联合检测可作为判断乳癌患者预后及指导制定治疗方案的手段之一。Objective :To study the relationship between the expression of bcl - 2,mn23 - h1 gene and lymph node metastasis in breast cancer cell. Methods:The expression of bel -2, nm23 - h1 protein was examined in 85 breast cancer patients and their norlnul breast tissue using immunohistochimical SP method. Results:The bel -2 over -expression was observed in 63.53% (54/85) of carcinoma tissues, it was associated with cancer cell lymph nodes metastasis ( LNM ) and TNM staging(P 〈 0.05). in 40.00% (34/85) of tumors, nm23 - h1 expressions were negtive,with correlated with the tmnor size,LNM and TNM staging(P=0. 032,0. 001and 0. 001,respectivly). Univariate analysis showed that bcl - 2 and nm23 - h1 expression were mainly influential factors for LNN(OR =6. 985 and OR= 0. 014, P 〈 0. 001 respectively). Concluslon:The bel - 2 and nm23 - h1 simultaneous determination may be useful for predicting of LNM risk and poor prognosis in patients with breast cancer and guiding therapeutic methods.
