两种序贯放化疗对广泛期小细胞肺癌治疗的对比研究

摘　　要：目的:探讨两种不同序贯放化疗方案对广泛期小细胞肺癌患者的治疗价值。方法:广泛期小细胞肺癌20例,随机分为放化疗组和夹心组2组,每组10例,化放疗组为4周期EP(Etoposide+Cisplatin)方案化疗后序贯放疗,夹心组为2周期EP方案化疗,序贯放疗,再序贯2周期EP方案化疗后比较两组的中位生存时间、1年和2年生存率、胸内复发率及治疗毒副反应。结果:全组中位生存时间为13.0个月,1年和2年生存率分别为50%和15%,其中化放疗组分别为14.0个月、60%和20%,夹心组分别为12.0个月、40%和10%。两组1年和2年生存率差异无统计学意义(P=0.49)。化放疗组胸内复发率为30%(3/10),夹心组胸内复发率为40%(4/10),差异无统计学意义(P=0.65)。两组治疗的毒副反应无统计学差异。结论:先化疗后放疗与夹心治疗对于广泛期小细胞肺癌患者的中位生存时间、1年和2年生存率、胸内复发率相似。Objective：To explore the value of two different sequential chelnotherapy and radiotherapy on treatment of patients with extensive stage small cell lung eaneer（SCLC）. Methods：Twenty cases of extensive stage SCLC patients divided into two groups randomly, 10 cases of each group,4 cycles of EP（Etoposide ＋ Cisplatin） regimen chemotherapy sequential thoracic radiotherapy as ehelnoradiotherapy（CRT） group,2 cycles of EP regilnen ehelnotherapy sequential radiotherapy further sequential 2 cycles of EP regilnen ehelnotherapy as sandwich（CRCT） group,eolnpare lnedian survival tilne, 1 - year and 2 - year survival rates,the incidence of intra - thoracic local failure and adverse reactions of the two groups. Results： For the whole group,the lnedian survival time was 13.0 months, 1 - year and 2 - year survival rates were 50 % and 15 % respectively, 14.0 lnonths, 60 % and 20 % and 12.0 months, 40 % and 10% of the CRT and CRCT group respectively, 1 - year and 2 - year survival rates showed no significant differenee（P =0.49） ,the incidence of intra - thoracic local failure was 30% in the CRT group and 40% in the CRCT group（P= 0.65） ,the adverse reactions of the two groups showed no significant difference. Conclusion：CRT group COlnpare with CRCT group showed no significant difference on the median survival tilne, 1 - year and 2 - year survival rates and the incidence of intra - thoracic local failure in patients with extensive stage SCLC.
