经内镜尼龙绳套扎联合高频电切治疗结直肠巨大息肉的临床分析

摘　　要：目的探讨内镜下尼龙圈套扎法联合高频电切治疗结直肠巨大息肉的应用价值。方法对2008年1月—2013年1月我院消化内科收治的85例结直肠巨大息肉(直径≥2 cm)患者采用结肠镜下尼龙圈结扎装置将尼龙圈套扎于大息肉根部,再行高频电凝切除治疗。结果本组85例共105枚亚蒂或粗蒂大肠息肉均一次性成功套扎并同时行高频电切除术,一次性切除息肉81枚(77.14%),分次切除24枚(22.86%),其中即刻出血2例(2.35%),立即给予内镜下电凝和(或)钛夹,即时止血,术后3例有蒂息肉渗血,原尼龙圈下方再次套扎1枚尼龙圈后出血停止。所有患者均未见迟发性出血、穿孔等并发症。术后1、3、6个月、1年复查胃肠镜,未见复发。结论内镜下尼龙圈联合高频电切治疗结直肠巨大息肉的方法安全有效。Objective To evaluate the efficacy and safety of endoscopic treatment with nylon endoloop combined with electroblation for large colorectal polyps. Methods A total of 85 patients with large colorectal polyps （diameter≥2cm） treated with endoscopic nylon endoloop combined with electroblation between January 2008 and January 2013 were analyzed retrospectively. Results All the 105 large colorectal polyps in the 85 patients were carried out successfully with nylon endoloop combined with electroblation. There were 81 polyps （77.14%） removed by one-time and 24 polyps （22.86%） removed step by step, including 2 cases （2.35%） of immediate bleeding treated with immediate endoscopic electric coagulation and/or titanium clamp for immediate control of bleeding, and 3 cases of postoperative bleeding form polyp controlled by another nylon ring below the original one. All the patients did not suffered from late complications such as hemorrhage and perforation. After 1 , 3, 6 and 12 month（s） , no recurrence happened. Conclusion Endoscopic nylon endoloop combined with electroblation is safe and effective on removal of large polyps.
