肝癌的“双灌注”治疗

摘　　要：目的 :观察肝动脉灌注化疗栓塞加经肠系膜上动脉或脾动脉间接性门静脉内灌注化疗药物 ,即“双灌注”的疗效。方法 :92例肝癌患者 ,采用Seldinger's技术 ,经右股动脉插入5FRH型或Yashiro导管 ,行肝动脉灌注化疗栓塞。同时A组58例经肠系膜上动脉 ,B组34例经脾动脉间接性门静脉内灌注化疗。结果 :“双灌注”治疗后1,2,3年的生存率分别为62 5% ,41 6 %和23 4%。A ,B组之间无明显差异。结论 :“双灌注”治疗对肝肿瘤的主要供血血管进行了化疗药物灌注 ,是介入治疗的一种新方法 ,疗效好 。Objective:To study the therapeutic effects of hepatic artery chemo-embolization,and through intra-arterial mesenterica superior infusion of anti-cancerous agents or lienalis infusion to portal vein indirectly as called as“double intra-arterial infusion.Method:92 cases of hepatic tumors,percutaneous intra-arterial femoralis 5F RH or Yashiro catheter by the Seldinger's technique,hepatic artery was infused chemo-embolization,and group A 58 cases intra-arterial mesenterica superior infusion of anti-cancerous agents or group B 34 cases lienalis infusion to portal vein indirectly.Results:The survival rates for 1,2,3 years were 62 5%,41 6%and 23 4%respectively .There were no significant different between group A and group B.Conclusion:“Double intra-arterial infusion of anti-cancerous agents”infused main supply artery of hepatic carcinoma.It is a new interventional treatment method,treatment effects are good,and have practical clinical usefulness in treating hepatic carcinoma.
