摘  要:目的 :进一步认识大承气汤对急性胰腺炎的治疗机理。 方法 :采用胰管内逆行注入牛磺胆酸钠方法造成大鼠急性重型胰腺炎模型 ,造模后用大承气汤、生理盐水进行治疗 ,观察造模治疗后 12h、2 4h动物血清中CD11a/CD18、淀粉酶及胰腺组织中TNF含量的变化和胰腺组织病理学的改变。 结果 :应用大承气汤治疗组的大鼠血中的可溶性CD11a/CD18表达、胰腺组织中的TNF含量、胰腺组织病理损害程度、胰酶血症的水平均较生理盐水治疗组明显下降或减轻。 结论 :认为大承气汤治疗急性重型胰腺炎的机理可能不仅在于其能促进胰酶的排出 ,还与其能减少粘附分子CD11a/CD18的表达 ,减少胰腺组织中PMNs的浸润程度 。To further investigate the mulitple therapeutic mechanism of Dachengqi Tang in severe acute pancreatitis Methods:Severe pancreatitis SD rat model was induced by retrograde injection of 5% sodium taurocholic acid into the pancreatic duct.The rats were randomly divided into two treatment groups:Dachengqi Tang and normal saline.The changes of soluble adhesion molecules CD11a/CD18,amylase in blood serum and pancreatic tissue TNF level in both groups at 12 and 24 hours after modeling were observed.The pathologic changes were also scored.Results:The expression of CD11a/CD18,the TNF pancreatic concentration,the degree of pancreatic injury,and blood amylase in Dachengqi Tang group were remarkable lower than those in saline group.Conclusion:The therapeutic mechanism of Dachengqi Tang in severe acute pancreatitis might be inhibiting the expression of adhesion molecules and reducing the infiltration of PMN into pancreatic tissue with a result of attenuating the pancreatic injury in addition to enforcing the excretion of pancreatic enzymes.
