消化道大出血的DSA检查及介入治疗

摘　　要：目的:探讨消化道大出血经导管动脉造影及介入治疗的临床价值。方法:对21例消化道大出血患者采用Seldinger法穿刺股动脉进行选择性动脉造影,明确出血部位后行动脉栓塞治疗,栓塞剂为明胶海绵颗粒和弹簧圈,1例患者行肠系膜下动脉造影后灌注立止血。结果:21例患者中,16例造影发现出血部位,经介入治疗出血停止,其中2例介入治疗后1周内发生再出血。结论:DSA在消化道出血中具有较高的应用价值,动脉栓塞或立止血灌注治疗消化道大出血方法简单、疗效确切。Objective To investigate the clinical value of DSA for digestive tract massive hemorrhage. Methods Totally 21 cases of acute gastrointestinal tract massive hemorrhage were included. All the cases adopted selective angiograph with Seldinger technique through femoral artery and then underwent arterial embolization with gelfoam and coils. One patient was treated with infusion of reptilase after superior mesenteric artery angiography.Results Bleeding sites were found in 16 of the 21 patients,and the bleeding was stopped after interventional therapy. There were 2 cases rebleeding within one week after interventional therapy.Conclusion DSA is of great value for the diagnosis and treatment of the acute massive gastrointestinal hemorrhage. Artery embolization or perfusion of reptilase is simple and effective for the treatment of the acute massive gastrointestinal hemorrhage.
