不同手术方式治疗翼状胬肉对角膜屈光状态的影响

摘　　要：目的探讨翼状胬肉切除联合自体角膜缘干细胞移植和翼状胬肉切除联合羊膜移植对角膜屈光状态的影响。方法将86例(86眼)翼状胬肉患者随机分为两组,每组43例,A组采用翼状胬肉切除联合自体角膜缘干细胞移植治疗,B组采用切除联合羊膜移植治疗,比较两组患者手术前后裸眼视力、角膜曲率和角膜散光度。结果两组患者术后裸眼视力、角膜曲率较术前提高(P<0.05),角膜散光度较术前减低(P<0.05),三项指标组间比较差异均无统计学意义(P>0.05)。结论翼状胬肉切除联合自体角膜缘干细胞移植和联合羊膜移植均为治疗翼状胬肉的有效的手术方法,临床可根据具体情况选择应用。Objective To investigate the effect of pterygium excision combined with limbal stem cell autografting and pterygium excision with amniotic membrane transplantation on corneal refractive status. Methods A total of 86 patients （86 eyes） with pterygium were randomly divided into two groups, 43 cases were in each group, group A were treated with pterygium excision combined with limbal stem cell autografting, group B were treated with pterygium excision with amniotic membrane transplantation, compared the uncorrected visual acuity before and after operation, corneal curvature and astigmatism of patients in two groups. Results The uncorrected visual acuity after operation and corneal curvature of patients were better than before operation （P 〈0.05）, corneal astigmatism were decreased （P 〈0. 05）, three in- dexes were compared between the two groups, there were not statistically significant （P 〉0.05）. Conclusion Pterygium excision combined with limbal stem cell autografting and pterygium excision with amniotic membrane transplantation are effective operation method in the treatment of pterygium, it can be used according to the specific situation of clini- cal application.
