结肠息肉病理分析及电切后复发、癌变相关因素分析

摘　　要：目的分析结肠息肉的内镜特征及电切后复发、癌变情况，比较息肉电切前后的组织病理学结果，了解息肉电切后复发及癌变相关影响因素。方法分析总结2009年11月30日～2010年12月1日在我院接受肠镜检查的结肠息肉患者的内镜特征，分析影响息肉恶变的因素；对比行内镜下息肉切除的患者电切前后组织病理学结果，统计电切后息肉再发率，分析再发及癌变的相关因素。结果息肉患者共412例，检出息肉646枚，息肉电切前后的病理学结果不符合的概率为35．2％；电切后息肉再发率为51．8％；多发性息肉、年龄〉50岁、息肉直径〉1．0om及腺瘤性息肉更容易再发；年龄〉50岁、左半结肠息肉、直径〉1．0cm、广基及分叶状息肉容易癌变。结论结肠息肉电切前后的病理学结果存在较大差异，活检可能漏诊癌变息肉；息肉电切后再发率高，应定期进行复查；结肠息肉好发于中老年人，腺瘤性息肉体积越大，位于左半结肠、广基及分叶状息肉其癌变率越高。Objective To analyze the characteristics of colonic polyps and endoscopic recurrence after transurethral resection, cancer cases, compared before and after polyp resection histopathology results, understand polyps and cancer recurrence after transurethral related factorsx. Methods From November 30, 2009 to December l, 2010 in our hospital colonoscopy colon polyps endoscopic features, analyzed the factord that affect the polyp; compared endoscopic polypec- tomy in patients with transurethral resection after histopathological results, statistics polyp recurrence after transurethral resection rate, analyzed cancer recurrence and related factors. Results Polyps in patients with a total of 412 cases, 646 detected polyps, polyps before and after transurethral resection pathology results did not meet a probability of 35.2%; polyp recurrence after transurethral resection rate was 51.8%; multiple polyps, age greater than 50 years old, polyps diameter 〉 1.0 cm and adenomatous polyp relapsed easier; aged 〉 50 years, left colon polyp diameter 〉 1.0 cm, sessile and lobulated easy cancerous polyps. Conclusion Colon polyps before and after transurethral resection pathology results are quite different, biopsy may be missed cancerous polyps; polypectomy then send rate, should be reviewed periodically; colon polyps occur in the elderly, the more volume adenomatous polyps big, located in the left colon, and lobnlated sessile polyp the cancer rate is higher.
