晚期胃癌组织中β-微管蛋白Ⅲ的表达与多西紫杉醇化疗疗效的关系

摘　　要：目的 探讨晚期胃癌组织中β-微管蛋白Ⅲ（TUBB3）的表达与多西紫杉醇化疗疗效的关系.方法 回顾性分析新疆医科大学附属肿瘤医院2008年1月-2011年6月收治的62例接受含多西紫杉醇一线化疗方案治疗的晚期胃癌患者的病理资料,应用免疫组织化学PV9000法检测肿瘤组织中TUBB3蛋白的表达,并对TUBB3蛋白的表达水平与化疗疗效及不良反应间的关系进行分析.结果 62例晚期胃癌患者总的化疗有效率（CR＋PR）为51.56％,TUBB3（＋）者的有效率及控制率均低于TUBB3（-）者,进展率高于TUBB3（-）者,差异有统计学意义（P＜0.05）;即TUBB3（-）表达的患者接受含多西紫杉醇化疗疗效优于TUBB3（＋）表达者.最常见的不良反应为骨髓抑制和消化道反应,给予对症支持治疗后耐受性良好,TUBB3（＋）组和TUBB3（-）组患者的毒副反应无明显差异.结论 本研究提示TUBB3蛋白的表达或许可以作为含多西紫杉醇化疗方案治疗晚期胃癌的疗效预测指标之一.尚需大样本前瞻性临床研究进一步证实.Objective To investigate the relationship between the expression of β - tubulin m （ TUBB3 ） in advanced gastric cancer tissues and the efficacy of docetaxel. Methods The pathological data of 62 patients admitted into our hospital from January 2008 to June 2011 who suffered with advanced gastric cancer and received docetaxel first -line chemotherapy were analyzed. The expression of TUBB3 in tumor tissues were examined by immunohistochemistry PV9000. The relationship between the level of TUBB3 protein, therapeutic efficacy and side effect was analyzed. Results Among 62 patients with advanced gastric cancer, the overall response rate （ CR ＋ PR） was 51.56% , where TUBB3 positive patients presented a lower effective rate hut a higher progressive rate than TUBB3 negative carriers, with statistically significant differences （ P 〈 0.05 ）. That is, TUBB3 negative patients brought better response to docetaxel treatment than the positive carriers. The most common side effects included inhibition of bone marrow and gastrointestinal reaction, which could be well tolerated after symptomatic supportive treatment. Conclusion The current study suggests the potential role of TUBB3 as a predictive indicator reflecting the therapeutic effects of docetaxel to treat advanced gastric cancer. Furthermore, largescale prospective evaluation is required.
