超声造影与磁共振动态增强成像对前列腺癌的诊断价值比较

摘　　要：目的探讨超声造影(CEUS)与磁共振动态增强成像(DCE-MRI)及其联合应用对前列腺癌的诊断价值。方法回顾性分析经系统穿刺活检术证实的75例前列腺疾病患者术前超声造影及MRI检查数据,结合病理组织学比较两种检查方法及其联合应用诊断前列腺癌的价值。结果 CEUS、DCE-MRI及二者联合对前列腺癌诊断的灵敏度分别为73.91%、82.61%和91.30%,特异度分别为76.92%、71.15%和57.69%,诊断准确率分别为76.92%、74.67%和68%,CEUS和DCE-MRI两种检查方法对前列腺癌的诊断价值差异无明显统计学差异(P>0.05);二者联合对前列腺癌诊断与单独应用一种诊断技术相比,差异均具统计学意义(P<0.05)。二者联合应用的ROC曲线下面积为0.921,高于单一检查的曲线下面积0.769(DCE-MRI)、0.754(CEUS),二者联合应用较单独应用一种具统计学意义(P<0.05)。结论 CEUS、DCE-MRI对前列腺癌的检测各具优势,两者的联合应用可有效提高诊断的灵敏度,为前列腺癌的检出提供有力支持。ObjectiveTo compare the value of contrast-enhanced ultrasound（CEUS） with dynamic contrast material-enhanced magnetic resonance imaging（DCE-MRI） for prostate cancer detection.Methods75 patients confirmed by systematic biopsy were retrospectively analyzed and all the patients underwent ultrasound of the prostate and MRI. The pathological results of biopsy were performed to evaluate the diagnostic value of CEUS and MRI.ResultsIn diagnosing prostate diseases, the sensitivity,specificity and accurate rate were 73.91%, 76.92%, 76.00% with CEUS imaging and 82.60%, 71.15%, 74.67% with MRI. The combination of the two was 91.30%, 57.69%, 68.00%.ConclusionCEUS, MRI in the differential diagnosis of prostate tumors have their own advantages, the combination of the two can provide strong support for prostate cancer early diagnosis and treatment.
