摘  要:目的:探讨脑梗死患者血浆瘦素水平与性别、年龄、时间及其与基因型的关系。方法对52例缺血性脑梗死患者和20例健康体检者分别采用酶联免疫分析法测定血浆瘦素水平,同时检测瘦素基因启动子区-2548G/a 的基因型和等位基因频率。结果脑梗死组的血清瘦素浓度明显高于对照组,差异有显著意义;脑梗死患者血清瘦素水平近于正态分布;血浆瘦素水平与脑梗死患者的年龄经相关分析:女 r =0.014;男 r =0.106。血浆瘦素水平与脑梗死时间经相关分析:r =0.098。缺血性脑梗死瘦素基因型aa和Ga在脑梗死男女患者血清瘦素水平进行分析,结果t值分别为0.261和1.891。两者均 P 〉0.05。结论脑梗死组的血清瘦素浓度明显高于对照组,差异有显著意义,;并近于正态分布;男性脑梗死患者血清瘦素水平变化幅度比较窄;而女性变化幅度宽;与脑梗死患者的年龄及卒中时间无相关性;瘦素基因启动子区-2548G/a 位核苷酸多态性。Objective to explore the relationship of serum leptin level in patients with cerebral infarction with gender, age, the time of cerebral infarction, and its relationship with genotype. Methods We measured serum leptin levels on 52 cases of patients with ischemic cerebral infarction and 20 cases of healthy controls respectively by using enzyme-linked immunoassay(elisa), then we extracted dna genome from peripheral blood leukocyte, using polymerase chain reaction (Pcr) combined with restrictive fragment length polymorphism(RFLP) analysis technology to detect the - 2548G/A genotype and allele frequency in gene promoter region of leptin. Results ① The leptin level in normal female is significantly higher than in male, the same as in patients group; With the same gender, the levels in both male and female patients were significantly above that of the same gender in normal controls; female in normal group showed the level of 1.89 times higher than male, while 1.71 times higher than male in patients group. ② the serum leptin level of patients with cerebral infarction approximated to normal distribution, and its 95% CI distribution were: 4.51 ~ 13.47 mg/l of male patients; 9.41 ~ 21.39 mg/l of female patient. ③the correlation analysis of serum leptin levels with age in cerebral-infarction patients was: female r = 0.014; male r = 0.106. ④the correlation analysis of serum leptin levels with the time of cerebral infarction was: r = 0.098. ⑤ the genotype of G - 2548a nucleotide polymorphism loci in leptin gene promoter region of patients with ischemic cerebral infarction turned out to be aa, Ga, GG. By analyzing genotype aa and Ga of serum leptin level in patients with cerebral infarction, their t value appeared to be 0.261 and 1.891, respectively. Both with P 〉 0.05.
