血清Ⅳ型胶原、透明质酸和层粘蛋白联合检测对肝纤维化诊断价值的探讨

摘　　要：目的:寻找肝纤维化敏感可靠的血清学指标。方法:对81例各种肝病患者联合检测血清IV型胶原(IV-C)、透明质酸(HA)、层粘蛋白(LN),并与40例健康人进行对照研究。结果:三项指标在急性肝炎和慢性轻型肝炎轻度升高,慢性肝炎重度和肝硬化三项指标均显著高于正常组(P<0.01)。结论:三项指标的联合检测可极大提高临床诊断的准确性和可靠性,对肝纤维化及肝硬化的诊断具有临床实用价值。Objective To establish sensitive and reliable serum index on liver fibrosis for early clinical diagnosis. Methods : The serum type IV collagen (IV-C), hyaluronate acid (HA) and laminin (LN) were joeutly detected in 81 cases with different hepatic diseases and compared with that in 40 healthy subjects. Results:The three indexes slightly higher in the groups of both acute and chronic mild hepatitis, in the groups of both chronic severe hepatitis and cirrhosis had obviously higher than those in the group of healthy subjects (P<0.01). Conclusion : The co-detection of three indexes can improve the accuracy and reliability of chinical diagnosis and provide a useful method for clinical diagnosis of hepatic fibrosis and cirrhosis.
