三段式硅橡胶导板在前牙区美学修复中的应用

摘　　要：目的探讨三段式硅橡胶导板在前牙美学修复中美学信息的传递及对牙体预备的指导作用。方法选择20例前牙美学修复患者,修复前制作诊断蜡型,其中12例患者根据诊断蜡型翻制硅橡胶导板,留取唇侧部分制作三段式导板指导临床操作中的牙体预备。结果采用硅橡胶导板的12例患者完全达到了诊断蜡型所设计的美学修复效果,并对最终的修复体效果满意。未采用硅橡胶导板的患者中5例认为最终修复效果体现了术前诊断蜡型的美学效果,2例患者通过沟通后与修改后,能接受临床修改过的最终修复效果,1例患者因修改影响修复体的功能和美观,重新制作后达到了预期的修复效果。结论三段式硅橡胶导板能准确传递诊断蜡型所设计的修复体的位置,外形等美学信息,从而指导临床进行精确的牙体预备,最终精准的再现诊断蜡型所体现的美学效果。Objective To investigate the function of three-stage silicon rubber template for aesthetic information transmitting and tooth preparation guider. Methods 20 patients of aesthetic anterior restoration were chosen. The diagnostic wax was prepared before restoration. The silicon rubber templates were fabricated according to the wax in 12 patients. The labial side of the template was used to make the three-stage silicon rubber template for tooth preparation conduction. Results 12 patients using the silicon rubber template were highly satisfied with the anterior aesthetic restoration that was completely in coincidence with the diagnostic wax. 5 in another 8 patients hold a view that the final restoration reflected the aesthetic effect of the previous diagnostic wax. 2 in another 8 patients were satisfied with the effect of the final restoration via dentist-patient communication and clinical grinding. However, 1 in another 8 patients was not satisfied with the grinded restoration, and finally pleased with the refabricated restoration. Conclusion Three- silicone template, which accurately transmitted diagnostic wax-designed prosthesis position, shape and other aesthetics information, could guide the accurate tooth preparation in dental clinic. The uhimate restoration could reappear the aesthetics of the diagnostic wax.
