骨环移植在纠正牙槽嵴骨量不足即刻种植中的应用

摘　　要：目的通过对缺牙区骨量不足患者实施骨环移植种植术,评价骨环植骨同期植入种植体的临床效果,讨论其应用价值。方法 9例患者通过自体骨移植术取骨,制备骨环植入缺牙区并同期植入Ankylos或Xive种植体共11枚,前牙5枚,前磨牙2枚,磨牙4枚,缺隙区可用自体碎骨或骨再生材料填补。龈下愈合,在这个阶段软组织要无张力缝合,并定期复诊。4-6个月后,经临床检查和影像学摄片评价骨环技术在改善缺牙区骨量不足时即刻种植的临床效果。结果 9例患者植入11枚种植体均无松动脱落情况,取骨区术后愈合良好,X线片示受植区种植体骨结合状况佳。8例患者受植区预后良好,牙槽嵴高度和厚度均达到理想水平。1例患者二期手术前受植区嵴顶部分暴露(种植体未暴露)。经治疗后效果不佳取出种植体。结论骨环技术为缺牙区三维骨量不足时即刻种植提供可能,并有效保证了缺牙区牙槽嵴三维骨量的恢复。与经典的二阶段骨增量种植术相比,骨环技术减少了种植治疗步骤,减少了植骨愈合期骨量损失,临床效果佳。不过需要注意对适应症的选取。Objective To implant bone rings in the case of advanced atrophy or jaw defectsi^and investigate success rate of immediate placement of implant with simultaneous bone grafting. Methods Choosing autologous bone graft for 9 patients,and fixing the bone rings,then placing rings and 11 implants,5 in anterior region,2 in premolar region,4 in molar region. Cavities should be filled with autogenous bone chips or bone regeneration material. Then an appropriate healing period, the soft tissue should be irritation-flee at that stage. Four to six months after surgery the radiographic image shows bone regeneration. Results All implants in receptor site perform no significant micromotion. The radio- graphic images reveal complete osseointegration. Well rehabilitation were acquired in 8 patients with bone volume gai- ning. Dentolaveolar crest bone was exposed and attachment gingiva recessed in 1 patient. The implant has been extracted out. Conclusion Bone ring technique allows placement of implants in the case of advanced atrophy or jaw defects by simultaneous bone grafting. Compared with the classic, two-stage augmentation using bone blocks,the bone ring tech- nique shortens the overall treatment time by several months, preventing any loss of volume occurring during the healing phase as a result of adaptation atrophy. Should take care of indications.
