涤痰汤对急性脑梗死患者胰岛素抵抗干预作用的临床研究

摘　　要：目的:观察涤痰汤治疗急性脑梗死患者的临床疗效及对胰岛素抵抗的作用。方法:将急性脑梗死患者60例随机分为治疗组(30例)与对照组(30例)。对照组采取常规西医治疗,治疗组在常规西医治疗基础上加用涤痰汤加减的汤药口服,疗程均为2周。观察两组临床疗效及治疗前后空腹血糖(FPG)、空腹血清胰岛素(FINS)、胰岛素敏感指数(ISI)的变化。结果:①治疗前后神经功能缺损程度评分两组均有显著性差异(P<0.05),治疗组治疗后总有效率为86.67%,对照组为93.33%,两组比较有显著性差异(P<0.05)。②两组患者治疗前后FINS、ISI均有改善(P<0.05),治疗组改善更为明显,与对照组治疗后比较有显著差异(P<0.05)。结论:涤痰汤治疗急性脑梗死,可显著改善患者的神经功能缺损,并明显改善胰岛素抵抗。Objective ： To observe the clinical efficacy and the effect of Ditan Decoction on insulin resistance in patients with a- cute cerebral infarction. Methods ： Sixty patients with acute cerebral infarction were randomly divided into treatment group （ 30 ca- ses）and control group（30 cases）. The patients in control group were treated with conventional western medicine, and those in treatment group added Ditan Decoction for 2 weeks. Clinical research observed the clinical therapeutic effect, the levels of fasting blood glucose（FPG）, fasting serum insulin（FINS）, insulin sensitivity index （ISI）before and after treatment in both treatment group and control group. Results ： ①There were significant improvements in both the neurofunction deficit scores after treatment（ P 〈 0. 05 ）. The total efficiency in the treatment group （86.67%）was more effective than that in the control group （93.33 % ）. The difference was significant（P 〈 O. 05 ）. ②The levels of FPG, FINS and ISI before and after treatment in the two groups were signifi- cant improved（ P 〈0.05 ）. After 2 weeks the improvement of levels of FINS and ISI in the treatment group were obviously lower than those in the control group（P 〈 O. 05 ）. Conclusion：The results suggest that Ditan Decoction not only improve neurofunction deficit scores significantly but also control the insulin resistance in acute cerebral infarction.
