摘　　要：目的:观察胸腔镜肺癌根治术患者围术期机体免疫与应激状态的变化情况。方法:选取于本院进行手术治疗的66例肺癌患者为研究对象,将其根据手术方式分为对照组(开胸肺癌根治术组)33例和观察组(胸腔镜肺癌根治术组)33例,然后将两组术前2d及术后3、10d时的红细胞免疫指标、细胞免疫指标及应激激素指标进行比较。结果:观察组术后3、10d时的红细胞免疫指标、细胞免疫指标均优于对照组,应激激素指标低于对照组,且观察组术后3d与术前2d及术后10d比较,差异均具有统计学意义(P<0.05)。结论:胸腔镜肺癌根治术患者围术期机体免疫与应激状态的波动均小于开胸手术的患者,其对患者的不良影响更小。Objective. To observe and study the change in states of body immune and stress state of patients with thoracoscopic radical resection of pulmonary carcinoma during the perioperative period. Methods=Totally 66 patients with pulmonary carcinoma were selected and divided into control group（open radical resection of pulmonary carcinoma group, n=33 cases） and observation group（thoraeoscopic radical resection of pulmonary carcinoma group, n= 33 cases）, then the erythrocyte immune indexes, cellular im- mune indexes and stress hormone indexes of two groups at second day before the surgery and at third and tenth day after the surgery were compared. Results=The erythrocyte immune indexes and cellular immune indexes of observation group at third and tenth day a{ter the surgery were respectively better than those ofcontrol group, the stress hormone indexes were all lower than those of control group, those indexes of ob- servation group at third day after the surgery were significant different from those at all second day before the surgery and at tenth day after the surgery （P^0.05）. Conclusion：The fluctuation of body immune and stress state of patients with thoracoscopic radical resection of pulmonary carcinoma during the periopera- tive period are smaller than those of patients undergoing open chest operation, and its negative influence is smaller.
