摘　　要：目的 :探讨周围型非小细胞肺癌 (NSCLC)的CT征像与 p2 1蛋白表达的相关性。 方法 :对 72例经手术病理证实的周围型NSCLC术前进行CT扫描 ,并用SP法对肿瘤标本 p2 1蛋白进行免疫组化染色。 结果 :⑴CT显示肿瘤的直径 >3cm及有深分叶征、棘状突起、纵隔淋巴结肿大者p2 1蛋白表达升高。⑵肺癌的 p2 1蛋白表达与肿瘤分化程度及组织学类型有关。结论 :周围型NSCLC的CT征像中肿瘤的直径 >3cm ,深分叶征、棘状突起及纵隔淋巴结转移与 p2 1蛋白表达密切相关 ,具有相对更高的恶性程度 。Objective To study correlation between CT appearance and p21 expression in peripheral non-small cell lung cancer(NSCLC).Method Seventy-two cases of surgically and pathologically confirmed peripheral NSCLC were examined with computerized tomography before operation.The p21 expression was assessed by immunohistochemical technique in paraffin embedded tumor samples with the streptavidin peroxidase conjugated method.Results:⑴p21 expression of tumors with diameter over 3cm was significantly higher than those of 3cm or less;and p21 expression of tumors with deep lobulation or spiculate protuberance and mediastinal lymph node enlargement was higher than those without.⑵p21 expression was correlated with the degree of tumor cell differention and histological typing.The CT appearance of peripheral NSCLC with diameter over 3cm,deep lobulation or spiculate protuberance and mediastinal lymph node enlargement were closely correlated with p21 expression,which indicates higher degree of malignancy.Conclusions:There are correlations between CT imaging and p21 expression of NSCLC,it might play a role in peripheral NSCLC prognosis.
