无水酒精-碘化油乳剂门静脉栓塞的实验研究

摘　　要：目的　研究无水酒精 碘化油乳剂的最佳配比及其在门静脉栓塞中的有效性和安全性。方法　健康的Sprague Dawley(SD)大鼠 90只 ,随机数字表完全随机化分成 9组 ,每组 10只 ,剖腹细针穿刺门静脉主干。每组以 0 5ml/kg的剂量分别缓慢注入 :单纯无水酒精 ,4∶1、3∶1、2∶1、1∶1、1∶2、1∶3、1∶4无水酒精 碘化油乳剂 ,单纯碘化油 (以下按顺序称为第 1～ 9组 )。分别于栓塞后第 3、7、2 1天处死大鼠 ,观察栓塞效果 ,根据栓塞灶占肝脏表面积分成无、轻度、中度、重度 4级 ,记录各等级大鼠只数 ,行Ridit分析。同时测定肝功能的变化。结果　栓塞效果第 1～ 5诸组与第 7～ 9诸组比较 ,P <0 0 5 ;第 1～ 3组与第 6组比较 ,P <0 .0 5 ;第 1、2组与第 6组比较 ,P <0 0 5。结论　以 3∶1的无水酒精 碘化油乳剂作门静脉栓塞 ,能达到与单纯无水酒精同样的栓塞效果 ,且更安全。Objective (1) To observe the safety and efficacy of the portal vein embolization with the ethanol lipodol mixture injection in rats. (2) To find the optimum ratio of ethanol and lipodol in the mixture and offer theoretical foundation for the treatment of hepatic carcinoma with the mixture transportal injection. Methods Ninety rats were divided into 9 groups with 10 rats per group. All the rats were treated with a mixture of absolute ethanol and lipodol in different ratios, which was slowly injected into the portal vein during open surgery. The dose of mixture was 0.5 ml/kg. In 3 days, 7 days, and 21 days after the injection, the rats were killed, the infarction areas of liver were compared and the change of liver function was determined as well. Results (1) 3 rats died in 6 hours after injection, 87 rats survived well. (2) Various degrees of infarction of the liver were obtained with the ethanol lipodol mixture in ratios over 1∶2 transportal injection, in which the degrees of infarction was same in groups of 3∶1, 4∶1, and absolute ethanol( P >0.05), the degrees of infarction in groups of 2∶1, 1∶1, and 1∶2 was not better than those in group of absolute ethanol( P <0.05). Conclusion (1) The ethanol lipodol mixture was an optimum embolic material used in the portal vein embolization. (2) As an embolic material used in the portal vein clinically, the 3∶1 ethanol lipodol mixture might be more efficient and safer than absolute ethanol.
