^(18)F氟代脱氧葡萄糖正电子发射断层成像术/计算机断层扫描显像在胆囊癌中的诊断价值

摘　　要：目的总结胆囊癌的氟代脱氧葡萄糖(18F-FDG)正电子发射断层成像术/计算机断层扫描(PET/CT)显像特征,提高胆囊癌早期诊断准确性。方法回顾25例疑似胆囊癌患者的18F-FDG PET/CT表现,对显像结果采用半定量和目测法分析,分别与单纯PET、CT、病理检查及临床结果进行对比研究。结果 25例胆囊疾患中恶性肿瘤21例(其中原发胆囊癌19例,胆囊癌术后复发2例),良性病变4例;胆囊床受侵及肝内转移14例、十二指肠受侵2例、骨骼等远处转移3例、腹腔及腹膜后淋巴结转移17例;PET/CT诊断胆囊癌的敏感性为95%、特异性为75%、准确性为92%。结论 18F-FDG PET/CT显像对胆囊癌的诊断及鉴别诊断、临床分期具有重要价值,并可准确监测疗效。Objective To summarize the imaging performance of gallbladder cancer with ＇SF-fluorodeoxyglucose （FDG） positron-emission tomography （PET）/CT and improve the diagnostic accuracy of gallbladder cancer with PET/CT. Methods Reviewed the appearance of 25 cases of suspected gallbladder cancer and analyzed imaging results using semi quantitatire and visualization, and compared the results between PET-CT and pathological examination respectively. Results In 25 cases of gallbladder disease, of which 21 cases of malignant tumor （of which 19 cases of primary gallbladder carcinoma, 2 cases of gallbladder cancer recurrence）, 4 cases of beingn lesions; Gaubldder bed invasion and intrahepatic metastasis in 14 cases. 2 cases of duoderal invasion. 3 cases of bone and other distaut metastasis. Abdominal cavity and retroperitoneal lymph mcle. Metastasis in 17 cases. The .sensitivity, specificity and accuracy of PET/CT diagnosis of gallbladder diseases were 95%, 75%, 92%. Conclusion The gallbadder cancer with 1SF-FDG PET/CT plays an important role in the diagnosis and differential diagnosis clinical stage of gallbladder cancer and can accurately monitor the efficaly.
