改良支气管动脉灌注化疗对中晚期肺癌患者疗效及生存质量的影响

摘　　要：目的探讨经支气管动脉微泵泵入灌注化疗对中晚期肺癌患者疗效及生存质量的影响。方法选取支气管动脉灌注化疗的中晚期肺癌患者60例,随机分为治疗组和对照组,治疗组给予经支气管动脉微泵泵入化疗药,泵入时间2 h;对照组给予经支气管动脉推注化疗药,时间30 min。治疗半年后进行临床疗效的评估,并在治疗前、治疗后应用欧洲癌症研究与治疗组织(EORTC)及生活质量核心问卷(QLQ-C30)对患者生存质量进行评估。结果对照组半年有效率60%,治疗组87%,2组比较有显著性差异(P<0.05)。2组患者治疗后生存质量评分均较治疗前显著好转(P均<0.05)。治疗后治疗组患者生存质量评分较对照组明显增加(P<0.05)。结论改良支气管动脉灌注化疗可以明显提高中晚期肺癌患者的近期疗效,改善生存质量。Objective It is to approach the influence of infusion chemotherapy by pumping with micro-pump through bronchial arterial on effective and quality of life in patients with advanced lung cancer. Methods Sixty patients with advanced lung cancer who treated by bronchial arterial infusion chemotherapy were randomly divided into treatment group and control group. Patients of treatment group were administered to chemotherapy drugs by pumping with micro-pump through bronchial arterial, pumping time was 2 hours. Patients of control group were administered to chemotherapy drugs by intravenous push through bronchial arterial, infusion time was 30 minutes. The curative effect was evaluated after treatment of half a year. The quality of life was assessed through applying EORTC and QLQ - C30 at pre-and post-treatment. Results The effective rate of half year was 60% in control group, and 87% in treatment group, there was significantly difference （P 〈 0.05 ）. The quality of life scores of both groups at post-treatment were obviously better than that at pre-treatment, there was significantly difference （P 〈 0.05 ）. The quality of life scores at psot-reatment in treatment group were signifi＇cantly better than those in control group, there was significantly difference （P 〈 0.05）. Conclusion The modified bronchial arterial infusion chemotherapy may improve short-term effect and quality of life in patients with advanced lung cancer.
