关节镜下双排锚钉缝合治疗肩袖损伤的临床疗效观察

摘　　要：目的探讨肩关节镜下双排锚钉缝合方法治疗肩袖损伤的手术方法、技巧及疗效。方法2010年10月—2012年6月，对28例肩袖损伤患者，术前均拍摄肩关节正位、冈上肌出口位X线片及MRI检查。所有患者在关节镜下肩峰成形与肩峰下滑囊切除后，采用肩关节镜下双排缝合锚钉缝合修复肩袖，使用美国加州洛杉矶大学（UCLA）功能评分标准进行术前和术后功能评估。结果28例患者获得6～14个月的随访，平均9．8个月。肩关节功能UCLA评分标准评估：术前平均为12．7分，术后为34．5分；其中优24例，良4例。术后疼痛完全消失有23例，偶感轻微疼痛或不适者有3例，剧烈运动或特殊动作疼痛者有2例，所有患者最终对手术效果满意。结论双排缝合锚钉固定是修复肩袖损伤的有效方法，该方法固定牢靠，有效增大腱骨接触面，促进肩袖一骨的愈合。Objective To investigate the surgical methods, techniques, and therapeutic effect of double-row anchor suture in the treatment of rotator cuff injury under arthroscope. Methods From October, 2010 to June, 2012, 28 cases of patients with rotator cuff injury were given preoperative shooting shoulder joint anteroposterior and supraspinatus muscle exports of X-ray films and MRI. All patients in the arthrescopic acromioplasty and subacromial bursa excision, the shoulder joint endoscopic double-row su- ture anchor suture repair the rotator cuff. And UCLA shoulder score standard was used to assess the preoperative and postoperative function. Results 28 patients were followed up for 6 to 14 months, an average of 9.8 months. The shoulder function evaluated by UCLA function score standard was 12.7 points preoperative and 34.5 points postoperative, on average; of which, excellent in 24 cases, good in 4 cases; after the operation, pain completely disappeared in 23 cases, mild pain or discomfort accidentally in 3 cas- es, strenuous exercise or special action pain in 2 cases. All patients eventually satisfied with the surgical results. Conclusion Dou- ble-row Suture anchor screw fixation is an effective way to repair a rotator cuff injury, it can fix firmly, effectively increase the contact surface of the tendon bone and promote the healing of the rotator cuff-bone.
